Title:Ocena epikardialnega maščevja in karotidnih arterij z magnetno resonanco pri otrocih s primarno arterijsko hipertenzijo

Abstract:Epikardialno maščevje je presnovno aktivno maščobno tkivo, ki se nahaja neposredno na površini miokarda. Epikardialno maščevje izloča bioaktivne molekule in ima potencialno vlogo pri razvoju primarne arterijske hipertenzije že v zgodnjem starostnem obdobju. S prospektivno magnetnoresonančno študijo smo primerjali volumen in debelino epikardialnega maščevja ter debelino intime medije pri otrocih in mladostnikih s primarno arterijsko hipertenzijo in pri zdravih kontrolah. Ugotovili smo, da imajo otroci s povišanim krvnim tlakom večji volumen in večjo debelino epikardialnega maščevja, ob tem pa tudi višjo debelino intime medije od zdravih vrstnikov. Vse spremenljivke kažejo pozitivno medsebojno korelacijo. Naš model je pokazal, da povečanje volumna epikardialnega maščevja za en 1 cm3 pomeni 9,5-krat večjo verjetnost za razvoj primarne arterijske hipertenzije pri otrocih in mladostnikih. Izmed vseh izbranih parametrov s hipertenzijo pri otrocih in mladostnikih najbolje korelira volumen epikardialnega maščevja. Sklepamo lahko, da je le-ta izmed izbranih dejavnikov najboljši napovedni dejavnik za možnost prisotnosti primarne arterijske hipertenzije pri otrocih in mladostnikih.

Title:Evaluation of epicardial adipose tissue and carotid arteries with magnetic resonance in children with essential arterial hypertension
Abstract:Epicardial adipose tissue is a metabolically active adipose tissue depot located directly on the surface of the myocardium. Epicardial adipose tissue secrets bioactive molecules and has a potential role in developing primary arterial hypertension at a very young age. In a prospective magnetic-resonance study, we evaluated the volume and thickness of epicardial adipose tissue and intima-media thickness in children and adolescents with elevated blood pressure and compared them with their healthy peers. Hypertensive patients have higher volume and thickness of epicardial adipose tissue and higher intima-media thickness compared to their healthy peers. All of the variables show a positive correlation. Our model shows that increasing epicardial adipose tissue volume for 1 cm3 results in a 9,5-times higher probability of developing hypertension. Out of all selected variables, the epicardial adipose tissue volume shows the best correlation with hypertension. We can conclude that epicardial adipose tissue is the best prognostic factor for the probability of arterial hypertension in children and adolescents.
