Abstract:
Kirurške instrumente uporabljamo med operativnim posegom za različne funkcije. Namen zaključnega dela je bil raziskati, kateri instrumenti se najpogosteje uporabljajo na področjih otorinolaringologije, oftalmologije, ginekologije, maksilofacialne kirurgije in visceralne kirurgije. V kvantitativni raziskavi smo v kliničnem okolju raziskali, kateri so najpogostejši posegi na določenih področjih. Instrumente smo razvrstili glede na uporabo pri teh najpogostejših posegih. Določili smo časovno zaporedje uporabe instrumentov in izdelali simulacijski model uporabe instrumentov. Ugotavljamo, da so najpogostejši posegi na področju otorinolaringologije septoplastika (32 %), adenoidektomija (26 %) in tonzilektomija (11 %). V oftalmološki operativni dvorani se največ opravi halacij (8 %), strabizmov (1 %) in sondiranj (1 %). V ginekološki operativni dvorani so najpogostejši posegi carski rez (38 %), histeroskopska ablacija polipa (18 %) in laparoskopska adneksektomija (15 %). Maksilofacialna operativa naredi največ posegov izdiranja zob in korenin (81 %), ekstraoralnih incizij (5 %) in zapor antrooralne komunikacije (5 %). Visceralna operativa pa izvede največ laparoskopskih holecistektomij (42 %), hernioplastik ingvinalne kile (enostransko) (28 %) in eksplorativnih laparotomij (14 %). Skupine, v katere razdelimo instrumente, so: instrumenti za prekinjanje tkiva, držalni instrumenti, prijemalni, pomožni, diagnostični in specialni instrumenti. Instrumente smo razdelili v naslednja časovna okna: začetek posega, sredina posega in čas do zaprtja rane. Simulacijski model smo izvedli v obliki posebne mape, ki vključuje prej pridobljene rezultate o funkcijah instrumentov in njihovi uporabi v določenem časovnem oknu. Instrumenti se med seboj razlikujejo po obliki, materialu, funkciji, predvsem pa je njihov izbor odvisen od operacijskega posega.

Title:
Simulation of surgical instruments usage
Abstract:
Surgical instruments are used during surgery for various functions. The purpose of the thesis was to investigate which instruments are most frequently used in the fields of otorhinolaryngology, ophthalmology, gynaecology, maxillofacial surgery and visceral surgery. With a quantitative research, we investigated in the clinical environment which are the most common procedures in certain fields. The instruments were classified according to their use in these most common procedures. We determined the chronological order of instrument use and developed a simulation model of instrument use. We established that the most common procedures in the field of otorhinolaryngology are septoplasty (32%), adenoidectomy (26%) and tonsillectomy (11%). In the ophthalmological operating room, chalazion surgeries (8%), strabisumus surgeries (1%) and probings (1%) are most frequently performed. In the gynaecological operating room, the most performed procedures are caesarean section (38%), hysteroscopic polyp ablation (18%) and laparoscopic adnexectomy (15%). Maxillofacial surgery performs the largest number of the following procedures: tooth and root extraction (81%), extraoral incision (5%) and closure of antro-oral communication (5%). The most common procedures in visceral surgery are laparoscopic cholecystectomy (42%), inguinal hernioplasty (unilateral) (28%) and exploratory laparotomy (14%). The groups into which we divide the instruments are: tissue-disruption instruments, holding instruments, gripping, auxiliary, diagnostic and special instruments. The instruments were divided into the following time windows: the beginning of the procedure, the middle of the procedure and the time until the wound closure. The simulation model was performed in the form of a special folder, which includes previously obtained results on the functions of the instruments and their use in a specific time window. The instruments differ in shape, material, function and, above all, their selection depends on the surgical procedure.
