Title:Intervencije za zmanjševanje posttravmatske stresne motnje po zdravljenju na enoti intenzivne terapije

Abstract:Izhodišča: Kritično bolni pacienti se po odpustu iz enote intenzivne terapije srečujejo s številnimi spremembami kognitivnih in psiholoških funkcij. Po izpostavljeni travmi pacienti lahko razvijejo posttravmatski stresni sindrom, ki slabi njihovo kakovost življenja. Z izvajanjem zgodnje rehabilitacije in multidisciplinarnim pristopom lahko izboljšamo kakovost življenja pacientov po odpustu in preprečimo ali obvladamo razvoj posttravmatskega stresnega sindroma. Namen diplomskega dela je predstaviti intervencije v enoti intenzivne terapije, ki pripomorejo k manjši pojavnosti posttravmatskega stresnega sindroma. Raziskovalne metode: V diplomskem delu smo uporabili metodo pregleda, analize in sinteze domače in strokovne literature. Uporabili smo deskriptivno metodo s pregledom naslednjih podatkovnih baz: PubMed, SageJournal, ScienceDirect. S PRISMA diagramom smo prikazali iskalno strategijo. Rezultati: S preprostimi intervencijami v času zdravljenja lahko pospešimo okrevanje pacienta in preprečimo razvoj posttravmatskega stresnega sindroma. Ugotavljamo, da intervencije vključujejo uporabo dnevnika, nadzor nad globino sedacije, psihoedukacijo in zgodnjo mobilizacijo. Potrebne so nadaljnje raziskave, ki bi natančneje opredelile vpliv posameznih intervencij na izboljšanje okrevanja in specifične dejavnike, ki vodijo v razvoj posttravmatskega stresnega sindroma. Diskusija in zaključek: Dokazana je učinkovitost določenih intervencij, kot so uporaba dnevnika v času zdravljenja, zgodnja mobilizacija, nadzor nad globino sedacije z vsakodnevnimi prekinitvami, vključenost družinskih članov in obvladovanje razvoja delirija.

Title:Interventions to reduce posttraumatic stress dissorder after treatment at the intensive care unit
Abstract:Background: Critically ill patients experience various changes in their cognitive and psychological functions after being discharged from the intensive care unit (ICU). After certain trauma exposure, patients can develop a post-traumatic stress disorder (PTSD) which significantly impairs their quality of life. By implementing early rehabilitation and a multidisciplinary approach, we can improve the quality of patients’ lives after discharge and prevent or manage the development of post-traumatic stress disorder. The objective of this diploma thesis is to present interventions in the intensive care unit that help reduce the incidence of post-traumatic stress disorder. Research methods: In the diploma work, we used these methods: literature review, content analysis and synthesis of domestic and academic literature. We used a descriptive method to review the following databases: PubMed, SageJournal, ScienceDirect. Furthermore, we used a PRISMA Flow Diagram to depict our search strategy. Results: Simple interventions during treatment can accelerate the patient’s recovery and prevent the development of post-traumatic stress disorder. We discovered that such interventions include the use of diaries, controlled depth of sedation, psychoeducation and early mobilization. However, further research is needed to specifically define the impact of individual interventions on the patient’s improving recovery and the specific factors that lead to the development of post-traumatic stress disorder. Discussion and conclusion: We have proven the effectiveness of certain interventions such as the use of a diary during treatment, early mobilization, controlled depth of sedation with daily interruptions, engagement of family members and management of delirium development.
