Title:Izdelava simulacijskega okolja za uporabo infuzijskih črpalk v enoti intenzivne terapije

Abstract:Povzetek Teoretična izhodišča: Enota intenzivne terapije je edinstveno in dinamično okolje, kjer je prisotno medicinsko osebje, ki nudi celostno obravnavo bolnika. Pri intenzivnem zdravljenju bolnikov navadno apliciramo hitro delujoča zdravila s pomočjo infuzijskih črpalk z brizgalkami. Raziskovalna metodologija in metode: Ustrezno literaturo smo pregledali s pomočjo iskanja po podatkovnih bazah zbirke PubMed in Cinahl. Za simulacijski del smo pripravili pogoje, ki čim bolj ustrezajo delovnemu okolju. Izvedli smo raziskavo o največkrat uporabljenih zdravilih in o tem, kako so v teh primerih nastavljene infuzijske črpalke. Rezultate poizvedbe smo aplicirali na simulacijo, ki je osrednja tema naloge. Uporabo infuzijskih črpalk smo simulirali v simulacijskem okolju Univerze v Mariboru, na Medicinski fakulteti. Prav tako smo ugotavljali, kakšni so tipični protokoli aplikacije zdravil na črpalkah in kako simulirati uporabo črpalke. Rezultati: V raziskavi je sodelovalo 28 zdravstvenih delavcev dveh intenzivnih terapij, povprečne starosti 38,5 leta in s povprečno delovno dobo 14,8 leta. Kot tri najpogosteje uporabljena zdravila, ki se aplicirajo preko infuzijskih črpalk, so navedli noradrenalin, dobutamin in lasix. Shematsko smo prikazali pravilno in varno aplikacijo zdravil preko infuzijskih črpalk, pripravo infuzijske črpalke ter pripomočke za aplikacijo terapije. Diskusija in zaključek: S pregledom literature in z delom v simulacijskem okolju smo ugotovili, da pravilna uporaba infuzijskih črpalk lahko zmanjša napake pri aplikaciji terapije, vendar je potrebni nadaljnji razvoj in izobraževanje kadra za njihovo uporabo.

Title:Creation of a simulation environment for the use of infusion pumps in the intensive care unit
Abstract:Abstract Theoretical basis: Intensive care unit is unique with its dynamic environment where interdisciplinary medical staff offers complete treatment to critically ill patients. When treating those patients, we often use syringe infusion pumps for delivering fast-acting therapy. Methodology: Relevant literature was reviewed by searching the PubMed and Cinahl databases. For the simulation part we have prepared conditions that are as similar as possible to the work environment. We inquired about the most commonly used medicines and how infusion pumps are set up in these cases. The results of the query were applied to the simulation, which is the central theme of the thesis. The use of infusion pumps was simulated in the simulation environment of the University of Maribor, Faculty of Medicine. We also identified typical pumping protocols for pumps and ways to simulate pump usage. Results: the study involved 28 healthcare professionals from two intensive care units, with an average age of 38.5 years and an average working age of 14.8 years. Noradrenaline, dobutamine and lasix are listed as the 3 most commonly used medicines administered via infusion pumps. We have shown schematically the correct and safe application of medicines via infusion pumps, the preparation of the infusion pump itself and the aids for the application of therapy. Discussion and conclusion: By reviewing the literature and working in a simulation environment, we have found that the use of infusion pumps can reduce errors in the application of therapy, but that further development and training of staff to use them are needed.
