Title:Bolnišnično zdravljenje odvisnih od alkohola v vzhodni Sloveniji

Abstract:Izhodišča: Z raziskavo smo želeli ugotoviti število in skupine hospitaliziranih bolnikov, odvisnih od alkohola, z zapleti in brez njih, ter njihove laboratorijske označevalce v letih 2002 in 2007 v psihiatričnih bolnišnicah vzhodne Slovenije. Metode: Študija je retrogradna. V raziskavo smo vključili vse osebe, odvisne od alkohola, ki so bile odpuščene v letih 2002 in 2007. Rezultati: V vseh bolnišnicah skupaj je bilo v letu 2002 odpuščenih 776 bolnikov, leta 2007 pa 672 bolnikov, odvisnih od alkohola. Med bolnišnicami obstajajo razlike v profilu obravnavanih bolnikov. Izstopa nizko število obravnavanih bolnikov z odtegnitvenim stanjem na psihiatričnem oddelku Univerzitetnega kliničnega centra Maribor. Izmed laboratorijskih preiskav vse bolnišnice kot označevalce odvisnosti določajo le gama-gluta-miltransferazo (GGT), aspartat-aminotransferazo (AST) in alanin-aminotransferazo (ALT) ter povprečni volumen eritrocitov (MCV). Vse srednje vrednosti označevalcev so pri odvisnih od alkohola z odtegnitvenimi simptomi v letu 2007 statistično značilno višje kot v letu 2002, razen vrednost MCV pri ženskah. Tudi deleži patoloških vrednosti vseh označevalcev so bili v letu 2007 višji. Zaključki: V bolnišnicah vzhodne Slovenije je bilo število skupno obravnavanih bolnikov, odvisnih od alkohola, leta 2007 statistično značilno manjše kot leta 2002. Zmanjšal se je tudi delež bolnikov, odvisnih od alkohola, v primerjavi z bolniki z ostalimi duševnimi motnjami. Odvisni od alkohola so bili glede na laboratorijske označevalce v letu 2007 telesno statistično značilno bolj prizadeti kakor v letu 2002. Razlike v patoloških vrednostih označevalcev niso bile tako izrazite, da bi nam v klinični praksi bistveno pomagale kot napovednik razvoja odtegnitvenih simptomov in alkoholnih bledežev. V nobeni od bolnišnic v diagnostičnem postopku ne določajo vrednosti transferina z zmanjšanim deležem ogljikovih hidratov (CDT) in glutamat-dehidrogenaze (GLDH), ki sta najbolj specifična označevalca.

Title:The hospital treatment of alcoholics in Eastern Slovenia
Abstract:Background: The purpose of this study was to evaluate the number and types of hospitalized alcohol dependents with and without withdrawal symptoms and their laboratory markers in the hospitals of Eastern Slovenia in the years 2002 and 2007. Method: This was a retrograde study. Included were all alcohol dependent patients who were discharged from hospitals in the years 2002 and 2007. Results: There were 776 alcohol dependent patients in the year 2002 and 672 in the year 2007 discharged from all hospitals. The hospitals differed from each other with respect to the profile of dismissed patients. Apparent is a low number of patients with withdrawal symptoms at the Department of Psychiatry of the University Medical Centre Maribor. In the complex of laboratory examinations in all three hospitals there were only gamma-Glutamyltransferase (GGT), Aspartat-Aminotransferase (AST) and Alanin-Aminotransferase (ALT) as well as mean corpuscular volume (MCV) determined. In the year 2007, all the mean values of alcohol dependency markers were statistically significantly increased as compared to the year 2002, except for MCV in women. Even the shares of pathological values were higher in the year 2007. Conclusions: The number of hospitalized alcoholics and all patients in psychiatric hospitals of the Eastern Slovenia was statistically significantly lower in the year 2007 than in 2002. Even the percentage of hospitalized alcoholics was decreased. The alcoholics hospitalized in 2007 were physically statistically significantly more affected than the alcoholics hospitalized in 2002.
