Abstract:V diplomskem delu smo predstavili vlogo in naloge medicinske sestre pri pacientu po transplantaciji ledvice, saj imajo medicinske sestre kot del zdravstvenega tima pri oskrbi pacienta po transplantaciji ledvice pomembne naloge. V prvem delu smo predstavili zgradbo in funkcijo ledvice, ledvično odpoved in načine zdravljenja končne ledvične odpovedi. V nadaljevanju smo podrobno predstavili transplantacijo ledvice kot eno izmed možnih oblik zdravljenja, ter transplantacijsko dejavnost v svetu in v Sloveniji, ter jo podkrepili z moralno pravnimi osnovami. Največji poudarek smo namenili zdravstveni negi pacienta po transpantaciji ledvice, saj se le ta razlikuje glede na to, koliko časa je preteklo od transplantacije. Opisali smo naloge medicinske sestre pri pacientu po transplantaciji ledvice v Centru za intenzivno terapijo, ki temeljijo na 24-urnem nadzoru hemodinamskega stanja pacienta in diagnostično-terapevtskih posegih. Predstavili smo tudi vlogo medicinske sestre na Kliničnem oddelku za nefrologijo, pri rehabilitaciji pacienta po operaciji, spodbujanju k samooskrbi ter opisali program zdravstvene-vzgoje pacienta pred odpustom v domačo oskrbo. Pri obravnavi pacienta po odpustu iz bolnišnice smo predstavili naloge medicinske sestre v Centru za transplantacijo ledvice, ki so predvsem zdravstveno-vzgojne in organizacijske narave. Na vseh področjih smo izpostavili najpogostejše negovalne diagnoze, glede na negovalne probleme in za le te načrtovali intervencije zdravstvene nege.

Title:The role of the nurse after kidney transplantation
Abstract:In this diploma thesis we presented the role and functions of nurses with the patient after renal transplantation, because the nurses carry out an important task as part of the health care team with patients after renal transplantation. In the first part we presented the structure and function of kidneys, kidney failure and treatment of final-stage renal failure. Further on, we presented detailed kidney transplantation as a possible form of treatment, and transplant activity in the world and in Slovenia, and supported it with the moral law basics. The greatest emphasis was given to health care of patient after kidney transplantation, since that in particular varies depending on how much time has passed since the transplant. We described the responsibilities of the nurse with a patient after renal transplantation in the Centre of intensive therapy, based on a twenty four hour monitoring of hemodynamic condition of the patient and the diagnostic-therapeutic procedures. We also presented the role of nurses in the Clinical Department of Nephrology, from the rehabilitation of patients after surgery, to self-supply, and described the health-education program of the patient before discharge to home care. In dealing with the patient after discharge from hospital, we presented the tasks of nurses at the Center for kidney transplants, which are primarily health-educational and organizational. In all areas we have highlighted the most common nursing diagnoses, according to the nursing problems and for those in particular we have planned the nursing interventions.
