Title:Vloga reševalca pri hitri sekvenčni intubaciji na terenu

Abstract:Hitra sekvenčna intubacija je način vzpostavitve dihalne poti, kjer je potrebno uporabiti zdravila, ki omogočijo lažjo intubacijo in zmanjšajo zaplete med posegom. Ker ta zdravila povzročijo globoko nezavest in prenehanje dihanja pacienta, je zelo pomembna usklajenost ekipe, ki hitro sekvenčno intubacijo izvaja. Pri izvajanju posega mora reševalec poznati poseg do potankosti, poznati mora tudi delovanje zdravil in aparatov, ki jih uporabljamo pri hitri sekvenčni intubaciji. Čeprav so študije dokazale, da imajo pravočasno intubirani pacienti boljšo prognozo in manj zapletov pri zdravljenju, se hitra sekvenčna intubacija ponekod v Sloveniji še vedno ne izvaja dovolj pogosto pri pacientih, kjer je indicirana. Raziskava, ki smo jo naredili med reševalci, zaposlenimi v reševalnih postajah, prehospitalnih enotah in enotah nujne medicinske pomoči na Štajerskem in Koroškem, je pokazala, da se reševalci hočejo izobraževati o intubaciji in hitri sekvenčni intubaciji, vendar nimajo vsi možnosti, ker nimajo organiziranih delavnic na to temo. Veliko reševalcev se izobražuje samovoljno, brez mentorjev ali strokovnega nadzora, kar lahko privede do napačnega učenja. Polovica anketiranih lahko nudi zdravniku dovolj dobro strokovno in časovno ustrezno pomoč pri izvajanju hitre sekvenčne intubacije, približno pri polovici anketiranih pa zdravnik ne bo deležen pričakovane asistence, kar lahko privede do hudih in življenje ogrožajočih zapletov med samim posegom.

Title:Paramedic's role at rapid sequence intubation in the prehospital setting
Abstract:Rapid sequence intubation is a way of establishing airways, where it is necessary to use drugs that facilitate intubation and reduce the complications of the intervention. Because these drugs cause deep unconsciousness and cessation of breathing, it is very important that coordination of the team that performs the rapid sequence intubation is well performed. The paramedic implementing the intervention should be familiar with the procedure in detail. Additionally, the paramedic should also be familiar with the drugs and devices that are used in rapid sequence intubation. Although studies have shown, that timely intubated patients have better prognosis and fewer complications from treatment, in Slovenia rapid sequence intubation is still not performed always frequently enough, where indicated. The survey, which we did during the paramedics, working in the ambulance stations, pre-hospital stations or emergency medical service in Štajerska and Koroška, it is shown that they want to educate about intubation and rapid sequence intubation, but they have not all the possibilities, they do not have organized exercises on this topic. Many paramedics are trained arbitrarily, without any mentors or peer review, which can lead to erroneous learning. Half of the respondents can provide sufficient medical expertise and timely assistance in the implementation of rapid sequence intubation, approximately half of the respondents doctor will not receive the expected assist, which can lead to serious and life threatening complications during rapid sequence intubation.
