Title:Uporaba ultrazvoka pri vstavitvi žilnih katetrov pri pacientu v urgentni ambulanti

Abstract:Uvod: Za lažjo vstavitev žilnih katetrov je priporočljiva uporaba ultrazvoka. Ultrazvok je namreč hitra in zanesljiva metoda pri oteženem venskem dostopu, s katero lahko prihranimo čas in se tako izognemo kasnejšim nepotrebnim vstavitvam centralnega venskega katetra in njihovim zapletom. Namen zaključnega dela je bil raziskati prednosti uporabe ultrazvoka pri vstavitvi žilnih katetrov pri bolniku v urgentni ambulanti v primerjavi s klasično slepo vstavitvijo žilnih katetrov. Metode: Uporabili smo metodo pregleda, analize in sinteze znanstvene literature ter metodo kompilacije in komparacije. Za izvedbo pregleda literature smo upoštevali PRISMA-smernice, članke pa smo ocenili s pomočjo Joana Briggs Institutes ocenjevalnega orodja. Za prikaz analize podatkov je bila uporabljena vsebinska analiza. Rezultati: V raziskavi smo prepoznali 177 člankov, od katerih smo v končno analizo vključili 12 člankov. Ugotovili smo, da je ultrazvočno vodena tehnika uspešnejša ter hitrejša od tradicionalne/slepe tehnike vstavitve žilnega katetra. S pomočjo ultrazvoka se zmanjšata čas vstavitve in število, pojavi pa se manj zapletov, zabeležena je nižja stopnja bolečine ter večje zadovoljstvo bolnikov. Razprava in zaključek: Žilni katetri so nujno potrebni za obravnavo kritično bolnih bolnikov in za aplikacijo intravenske terapije. Za lažjo in hitrejšo nastavitev intravenskega dostopa si lahko pomagamo z ultrazvokom, ki ima trenutno svoje mesto predvsem pri urgentnih stanjih, neizkoriščena pa je uporaba ultrazvoka v vsakdanji rabi.

Title:The use of ultrasound in the insertion of vascular catheters in a patient in the emergency room
Abstract:Introduction: Ultrasound is recommended to facilitate the insertion of vascular catheters. Ultrasound is a fast and reliable method for difficult venous access, which can save time and avoid subsequent unnecessary central venous catheter insertions and their complications. The aim of our study was to investigate the advantages of ultrasound guided insertion of vascular catheters in the emergency department compared to blind insertion. Methods: We used the method of review, analysis and synthesis of the scientific literature, as well as the method of compilation and comparison. PRISMA guidelines were followed to conduct the literature review, and the articles were assessed using the Joan Briggs Institutes assessment tool. Content analysis was used to demonstrate the data analysis. Results: The study identified 177 articles, of which 12 were included in the final analysis. We found that the ultrasound guided technique is more successful and faster than the traditional/blind insertion technique. Ultrasound guided insertion reduces insertion time and the number of attempts, which results in fewer complications, less pain and higher patient satisfaction. Discussion and conclusion: Vascular catheters are essential for the management of critically ill patients and for the administration of intravenous therapy. Ultrasound can be used to facilitate and speed up the set-up of the intravenous access, which currently has it's place mainly in emergency situations, but there is an untapped use of ultrasound in everyday use.
