Abstract:
Uvod: Simulacije so se v zadnjih letih v laparoskopski kirurgiji zelo razširile. Okolje, kjer se izvajajo simulacije, nam omogoča, da pridobimo znanje, na pridobljenih osnovnih znanjih gradimo in s spretnostmi začetnike pripravimo na delo z bolniki. Osredotočili smo se na tehnične podrobnosti z namenom zagotavljanja varnosti bolnika. Metode: Prikazali smo funkcionalne značilnosti različnih endoskopskih kanil in trokarjev ter izdelali simulacijo uporabe kanil, ki bi na področju perioperativne zdravstvene nege pripomogla k izobraževanju in tehnikam rokovanja s kanilami in trokarji. Z vso to opremo smo prikazali vstavljanje kanil in tehnike vstopa kanil ter s tem pridobili željene rezultate. Rezultati: V sklopu simulacij smo prikazali različne vstopne tehnike ter funkcionalnost različnih kanil in trokarjev v laparoskopski model. V laparoskopiji se pogosteje uporabljajo trokarji z rezilom in optični trokarji za enkratno uporabo. Prednost trokarja z rezilom za enkratno uporabo je funkcija obturatorja, kateri ima vzmetni mehanizem, ki ob uporu in predrtju strukture delno umakne in izpostavi ostro konico rezila. Razprava in sklep: Poznavanje značilnosti kanil in trokarjev je zelo pomembno pri izvedbi laparoskopskega ali artroskopskega operativnega posega, zato vidimo simulacijo na simulacijskem modelu kot odlično priložnost za pridobivanje izkušenj in znanja ter se tako lažje izognemo nastanku napak in poškodb pri resničnem delu na bolniku. Izobražen kader je namreč tisti, ki je potreben za kvalitetno izvajanje dela v zdravstveni organizaciji.

Title:
Functional analysis of endoscopic cannulas and trocars
Abstract:
Introduction: Simulations have become very widespread in laparoscopic surgery in recent years. The environment where the simulations are performed allows us to acquire knowledge, build basic knowledge and skillfully prepare beginners to work with patients. We focused on the technical details in order to ensure patient safety. Methods: We presented the functional characteristics of various endoscopic cannulas and trocars. Simulation of the cannulas use was made, which would contribute to the education and handling with cannulas and trocars in the field of perioperative nursing. With all this equipment, we demonstrated cannula insertion and cannula entry techniques and then collect the desired results. Results: As part of the simulations, we presented different entry techniques and the functionality of different cannulas and trocars in the laparoscopic model. Bladed trocars and disposable optical trocars are more commonly used in laparoscopy. The advantage of the trocar with a disposable blade is the function of the obturator, which has a spring mechanism that partially retracts and exposes the sharp tip of the blade when it resists and punctures the structure. Discussion and conclusion: Knowing the characteristics of cannulas and trocars is very important when performing laparoscopic or arthroscopic surgery, so we recognize the study on the simulation model as an excellent opportunity to gain experience and knowledge and virtually avoid errors and injuries in true work on the patient. Because educated staff is the one who is needed for quality work in a health care organization.
