Title:Odnos, poznavanje in zaznavanje zgodnjih znakov demence pri starostniku v ambulanti družinske medicine

Abstract:Uvod: Demenca predstavlja pomemben izziv za zgodnje odkrivanje bolezni na primarnem nivoju zdravstvenega varstva. Zgodnje odkrivanje je pomembno za kakovostno oskrbo obolelega in njegovih svojcev. Ključno vlogo za zgodnje odkrivanje in oceno kognitivnega upada ima zdravstveni tim družinske medicine, saj ima redne stike z večino starejše populacije. Metode: Uporabili smo kvantitativno metodologijo in deskriptivno metodo dela raziskovanja. Izvedli smo presečno raziskavo, v katero smo vključili 114 članov zdravstvenega tima družinske medicine. Za zbiranje podatkov smo uporabili metodo anketiranja. Rezultate smo analizirali in statistično obdelali z uporabo deskriptivnih statističnih metod. Za ugotavljanje statističnih razlik smo uporabili Mann-Whitney U test in Kruskal-Wallis enostransko analizo variance. Rezultati: Raziskava je pokazala, da je poznavanje demence med člani zdravstvenega tima družinske medicine slabo. Ugotovljena statistično pomembna razlika o poznavanju demence je bila pri starosti (p = 0,02), stopnji izobrazbe (p = 0,000), delovnem mestu (p = 0,000), skupni delovni dobi (p = 0,000) in delovni dobi na sedanjem delovnem mestu (p = 0,008). Rezultati so pokazali, da imajo člani tima družinske medicine pozitiven odnos do zgodnjega odkrivanja bolezni in dobro poznajo zgodnje simptome demence. Vidik skrbi za obolelega z demenco v ambulanti družinske medicine se je izkazal za težavnega. Anketirani si želijo boljšo dostopnost do podpornih služb in večjo prepoznavnost le-teh. Razprava in sklep: Zgodnje odkrivanje in diagnosticiranje demence pripomore k lažjemu zdravljenju in kakovosti življenja obolelega ter njegovih svojcev. Za izboljšanje kakovosti zdravstvene oskrbe obolelih z demenco je potrebno razumeti stališča, odnos, poznavanje bolezni in ovire za zgodnje prepoznavanje demence v ambulanti družinske medicine.

Title:Relation, knowledge and detection of early signs of dementia at eldery by the primary health care team
Abstract:Introduction: Dementia is an important challenge for an early detection of the first signs of the disease on the primary level of health care. Early detection is significant for a quality care of a sick person and his or her family. A crucial role of early detection and cognitive decrease assessment belongs to the health team of family medicine, since it has frequent contacts with the majority of the elderly population. Methods: We used the quantitative methodology and descriptive method of a research. We conducted a cross observational study in which were included 114 members of health team of family medicine. A method of surveys was used to gather the data. The results were analysed and statistically processed with the use of descriptive statistics. The findings of statistical differences were established with the Mann-Whitney U test and Kruskal-Wallis one-way analysis of variance. Results: The research has shown that dementia knowledge among the health team of family medicine is poor. A statistically important difference in dementia knowledge was in variables age (p = 0,02), level of education (p = 0,000), workplace (p = 0,000), total years of service (p = 0,000) and years of service at the current workplace (p = 0,008). The results have indicated that the members of health team of family medicine have a positive attitude towards the early detection of the disease and know the early symptoms of dementia well. The view of a care for a person with dementia has proven to be difficult in the family medicine clinic. The interviewees wish a better accessibility of support services and their bigger recognisability. Discussion and conclusion: An early detection and diagnose of dementia aid in easier treatment and quality of the patient's life and his family. An improvement in the quality of healthcare for patients with dementia is important to understand points of view, the attitude, the knowledge of the disease and obstacles for early detection of dementia in the family medicine clinic.
