Title:PSIHOLOŠKA PARADIGMA UVAJANJA INZULINSKE TERAPIJE

Abstract:Teoretična izhodišča: Pacienti s sladkorno boleznijo, ki potrebujejo inzulinsko zdravljenje doživljajo strah in negotovost, zaradi negativnih vidikov inzulinske terapije kot so: hipoglikemija, strah pred iglami, bolečina ob zbadanju z iglo in dvomi glede doziranja ter časa aplikacije inzulina. V kombinaciji z možnostjo pojava akutnih in kroničnih zapletov, ti dejavniki predstavljajo psihosocialno breme za diabetike. Namen diplomskega dela je bil izpostaviti psihosocialni vidik uvajanja inzulinske terapije, ugotoviti s kakšnimi težavami se srečajo diabetiki, ko jim je prvič predpisana inzulinska terapija in predstaviti vlogo medicinske sestre v procesu uvajanja inzulinske terapije. Metodologija raziskovanja: Metodo deskripcije z analizo domače in tuje literature smo uporabili za teoretični opis sladkorne bolezni, inzulinske terapije in smernic za uvajanje inzulinske terapije z upoštevanjem psihosocialnega vidika. Za empirični del diplomskega dela smo uporabili kvantitativno raziskovanje z metodo anketiranja. Ugotavljali smo, s kakšnimi psihosocialnimi težavami, strahovi in dilemami se soočajo pacienti, ko jim prvič uvedemo inzulin. V čakalnici diabetološke ambulante UKC Maribor smo anketirali 50 pacientov, ki si aplicirajo inzulin. Rezultati: Ugotovili smo, da je bilo 60 % anketiranih ob uvedbi inzulinske terapije splošno prestrašenih in zaskrbljenih, 66 % jih je uvedbo inzulina videlo kot poslabšanje bolezni, 60 % jih je bilo strah hipoglikemije, 100 % jih je ob uvedbi inzulina dobilo dovolj informacij s strani zdravstvenega tima in 58 % anketiranih si najraje aplicira inzulin v prostoru, ki omogoča zasebnost. 96 % anketiranih se zaradi potrebe po inzulinu ne počuti diskriminirano. Sklep: Sladkorna bolezen in inzulinska terapija zahtevata velike spremembe v življenjskem slogu diabetika. Diabetiki si želijo, da bi bilo več pozornosti pri obravnavi namenjeni psihološki podpori. Medicinska sestra pri obravnavi pacienta s sladkorno boleznijo deluje kot vzgojiteljica in pacientov zagovornik, ter upošteva psihosocialno breme sladkorne bolezni in negativne vidike inzulinske terapije. V obravnavi spoštuje pacientovo zasebnost, pravico do samoodločanja in pravico do izbire lastnega življenjskega sloga.

Title:PSYCHOLOGICAL PARADIGM OF INSULIN INITIATION
Abstract:Theoretical starting points: Patients with diabetes who need insulin therapy are experiencing fear and doubt, due to negative aspects of insulin therapy, such as: hypoglycemia, fear of needles, pain when sticking with a needle and doubts about dosing and time of insulin administration. In combination with a chance of developing acute and chronical complications, those factors present a psychosocial burden for diabetics. The purpose of the thesis was to highlight the psychosocial aspect of insulin therapy initiation, to find out what problems diabetics have to overcome in the process of insulin therapy initiation and to present the role of a nurse in this process. Research methodology: We have used a method of description with analysis of domestic and foreign literature to theoretically present diabetes, insulin therapy and guidelines for insulin initiation by acknowledging psychosocial aspects. For the empiric part of the thesis we have used quantitative research with method of surveying. We were researching the psychosocial problems, fears and dilemmas that the patients encounter when first prescribed insulin. We have surveyed 50 diabetics on insulin in the waiting room of the diabetes clinic in UKC Maribor. Results: We have found that 60 % of the surveyed were frightened and concerned upon insulin therapy initiation, 66 % saw this development as a worsening of the disease and 60 % were afraid of hypoglycemia, 100 % of participants received enough information from their caregivers and 58 % prefers to administer insulin in a space that enables privacy. 96 % of the surveyed do not feel discriminated due to their need for insulin. Conclusion: Diabetes and insulin therapy demand great changes in lifestyle. Diabetics wish for more attention to be given to psychological support. In treatment of patients with diabetes, a nurse acts as a teacher and as a patient’s advocate. She acknowledges the psychosocial burden of diabetes and negative aspects of insulin therapy. At her work she respects the patient’s privacy, right of self-determination and the right of free choice of lifestyle.
