Title:SMERNICE ANTIBIOTIČNE ZAŠČITE PRED OPERATIVNIM POSEGOM V UKC LJUBLJANA

Abstract:V Sloveniji so za obravnavo pacienta pri kirurškem posegu glede antibiotične zaščite od leta 2006 izdelane smernice, po katerih bi se morali izvajati vsi kirurški posegi na vseh kirurških oddelkih. Smernice namreč zagotavljajo strokovno priporočilo oskrbe pacienta tako iz zdravstvenega kot tudi finančnega vidika. V magistrski nalogi smo ugotavljali skladnost s smernicami na 8 kirurških enotah v UKC Ljubljana. Preverjali smo, ali je bolnik pred kirurškim posegom dobil antibiotik, ali sta bila antibiotik in doza ustrezna, ali je dobil antibiotik ob ustreznem času in ustrezno število odmerkov. Preverjali smo, ali število odmerkov vpliva na ležalno dobo. Predpostavljali smo, da so posamezni kirurški oddelki antibiotike aplicirali skladno s smernicami. Rezultati raziskave kažejo, da je bila skladnost s smernicami samo 28 % ob predpostavki, da se smernic držimo 100 %. Antibiotično zaščito so aplicirali v 86,8 % in to kar v 95 % ustrezen antibiotik. Največ neskladnosti je bilo pri pregledu števila odmerkov antibiotične zaščite (45,8 %), saj so v večini primerov le-te aplicirali prekomerno, kar sicer ni vplivalo na število hospitaliziranih dni pacienta. Skladnost je treba popraviti predvsem pri upoštevanju ustreznega števila apliciranih odmerkov antibiotika. Predpisovanje in aplikacija antibiotične zaščite skladno s smernicami sta odvisna od izobraževanja zdravstvenega osebja, nadzora, povratnih informacij in ne nazadnje od dobre organizacijske in posledično varnostne kulture. Zelo pomembna je dobra medsebojna komunikacija.

Title:GUIDELINES FOR ANTIBIOTIC PROPHYLAXIS IN SURGERY AT UNIVERSITY MEDICAL CENTRE LJUBLJANA
Abstract:In 2006, Slovenia introduced guidelines on antibiotic protection during the treatment of patients undergoing surgical procedures, and all surgical procedures in all surgical departments should comply with these guidelines. The Guidelines ensure a professional recommendation for patient care both from both the health and the financial perspectives. In the Masters’ Thesis, we ascertained the compliance with the guidelines in eight surgical units at the University Medical Centre UKC Ljubljana. We examined whether the patient was given an antibiotic prior to surgery, whether the antibiotic and its dosage were appropriate, whether the antibiotic was administered at the appropriate time and in appropriate number number of doses. We also examined whether the number of doses affects the duration of hospital stay. The hypothesis was that individual surgical departments administered antibiotics in accordance with the guidelines. The survey results show that the compliance with the guidelines was only 28%, provided that the compliance with the guidelines is 100%. Antibiotic protection was administered in 86.8% cases, and in as many as 95% the appropriate antibiotic was administered. The greatest amount of non-compliance was observed in the review of the number of doses of antibiotic protection (45.8%), since in most cases it was over-administered, which, however, did not affect the number of days of hospitalization for the patient. Compliance should be most improved in particular with regard to the appropriate number of doses of antibiotics administered. Prescribing and administrating of antibiotic protection in accordance with the guidelines depend on the education of health personnel, supervision, feedback, and, last but not least, on an overall good organizational and consequently safety climate. Good interpersonal communication plays a great role.
