Title:TEHNOLOGIJA ZUNAJTELESNEGA KRVNEGA OBTOKA IN MONITORINGA MED OPERACIJO NA ODPRTEM SRCU

Abstract:V diplomskem delu smo analizirali tehnologijo zunajtelesnega krvnega obtoka (ZTKO), ki jo uporabljamo med operacijo na odprtem srcu. Ugotavljamo, da ni bistvene razlike v opremi, ki jo uporabljajo trije kardiokirurški centri v Sloveniji (Maribor, Ljubljana in Izola). Valjčne črpalke imajo še vedno prednost pred centrifugalnimi, ki se pri nas uporabljajo le v zunajtelesni membranski oksigenaciji (ECMO). Za zaščito miokarda uporabljamo krvavo anterogradno in retrogradno kardioplegijo v razmerju 4:1. Monitoring plinov v arterijski krvi izvajamo v kardiokirurškem centru Maribor s sistemom CDI-500, ki kontinuirano meri delne tlake plinov in pH vrednosti. Ostala dva kardiokirurška centra uporabljata laboratorijsko POCT (Point Of Care Testing) merjenje. Iz pregleda literature ugotavljamo, da mehanizmi vzdrževanja PaCO2 in pH med hipotermijo še vedno niso dovolj jasni in da so razlike med alfa-stat in pH-stat pristopom še vedno nedorečene. V vseh kardiokirurških centrih po Sloveniji se uporablja alfa-stat pristop. Za učenje s simulacijami na področju zunajtelesne oksigenacije in cirkulacije predlagamo odprtokodni program ECMOjo. Našli smo samo en fizični simulator ZTKO – ORPHEUS. Tehnologija ZTKO, ki se uporablja v Sloveniji, se ne razlikuje od sodobne tehnologije, ki jo uporabljajo drugod po svetu.

Title:Extracorporal circulation technology and monitoring during open heart surgery
Abstract:In the thesis we have analyzed the technology of extracorporal circulation (ECC), which is used during open-heart surgery. We find that there is no significant difference in equipment used in all three cardiac centers in Slovenia (Maribor, Ljubljana and Izola). Roller pumps are still preferred than centrifugal pumps, which are used by us only in the extracorporeal membrane oxygenation (ECMO). To protect the myocardium we are using anterograde and retrograde blood kardioplegic in a 4:1 ratio. Monitoring of arterial blood gases is performed in the cardiac center of Maribor with CDI-500 system, which continuously measure the partial pressure of gases and pH. The other two cardiac center are using laboratory POCT (Point Of Care Testing) measurement. A review of the literature, we find that the mechanisms of maintaining PaCO2 and pH during hypothermia are still not sufficiently clear and that the differences between alpha-stat and pH-stat approach is still unknown. In all cardiac centers in Slovenia is used alpha-stat approach. For learning extracorporeal circulation and oxygenation with simulation we suggest free of charg program ECMOjo. We found only one physical simulator of ECC - ORPHEUS. ECC technology used in Slovenia is no different from modern technology used by the rest of the world.
