Title:Zagotavljanje udobja za kritično bolne paciente v enoti intenzivne terapije

Abstract:Uvod: Koncept udobja je kompleksen, prehoden in večdimenzionalen, presega le odsotnost bolečine. Namen intenzivne terapije je zagotavljanje kakovostne oskrbe hudo poškodovanim pacientom. Raziskava proučuje stališča medicinskih sester o zagotavljanju udobja, vpliv tipa enote in izkušenj ter ugotavlja najpogostejše vire neugodja. Metode: Uporabljena je bila deskriptivna presečna raziskava z metodo anketiranja. Analiza podatkov je vključevala deskriptivno in inferenčno statistiko s programom SPSS IBM 23.0, vključno z Mann-Whitney U testom. Rezultati: Najpogostejši vir neugodja je pogrešanje ljubljenih oseb (x̄ = 7,83, SD = ±2,18). Pogosti viri neugodja so še motnje spanja, bolečina, tesnoba in občutek odvisnosti. Izkušnje medicinskih sester vplivajo na zagotavljanje udobja, predvsem pri zagotavljanju spanja ponoči, preprečevanju žeje, udobnem nameščanju pacientov in obvladovanju bolečine. Medicinske sestre največ pozornosti namenjajo odpravljanju bolečine, občutka mrazu, toplote, žeje ter zmanjševanju hrupa in umetne svetlobe ponoči. Razprava in sklep: Intenzivna terapija je kompleksno okolje, kjer medicinske sestre igrajo ključno vlogo. V prihodnosti bi bilo smiselno raziskati skladnost mnenj medicinskih sester in pacientov ter razviti orodje za ocenjevanje udobja v Sloveniji, kar bi pomembno prispevalo k klinični praksi.

Title:Ensuring comfort for critically ill patients in the intensive care unit
Abstract:Introduction: The concept of comfort is complex, transient and multidimensional, going beyond the absence of pain. The aim of intensive care is to provide quality care to severely injured patients. This research aims to explore nurses' views on the ensuring of comfort to ICU patients, the impact of unit type and nurses' experience on the ensuring of comfort, and to identify the most common source of discomfort from nurses' perspective. Methods: Data analysis included descriptive and inferential statistics using SPSS IBM 23.0, including Mann-Whitney U test. Results: The most common source of discomfort is missing loved ones (x̄ = 7.83, SD = ±2.18). Other common sources of discomfort are sleep disturbance, pain, anxiety and feelings of dependency. Nurses' experience has an impact on comfort provision, especially in ensuring sleep at night, preventing thirst, positioning patients comfortably and pain management. Nurses focus on pain management, cold, heat, thirst and reducing noise and artificial light at night. Discussion and conclusion: In the future, it would be worthwhile to investigate the concordance of nurses' and patients' views and to develop a tool for assessing comfort in Slovenia, which would contribute significantly to clinical practice.
