Title:Uporaba adrenalina pri srčnem zastoju

Abstract:Uvod: Srčni zastoj predstavlja velik problem v zdravstvu, saj se njegova pojavnost neznatno viša; za njim umre veliko ljudi. Ključno pri srčnem zastoju je njegova prepoznava in znanje zdravstvenih delavcev, saj sta hitra defibrilacija in zunanja masaža srca ključni aktivnosti za povrnitev spontanega krvnega obtoka (ROSC). Namen diplomskega dela je bil raziskati učinke adrenalina pri srčnem zastoju. Metode: Pregled literature na temo učinkov adrenalina pri pacientih s srčnim zastojem smo iskali v naslednjih podatkovnih bazah: CINAHL, ScienceDirect, PubMed in Cochrain library. Vso zbrano literaturo smo iskali in prikazali s PRISMA diagramom. Rezultati: Od 400 najdenih člankov smo jih 8 vključili v končno analizo. Uspeli smo ugotoviti pozitivne in negativne učinke adrenalina. Najpomembnejše ugotovitve so pokazale, da adrenalin v manjših odmerkih in odmerkih z daljšim časovnim oknom vpliva na hitrejšo povrnitev spontanega krvnega obtoka in ugoden nevrološki izid. Razprava in zaključek: Ugotovili smo, da adrenalin ob hitri aplikaciji in v ustreznih dozah ter pravem času dajanja pozitivno vpliva na povratek spontanega srčnega ritma in ugodne nevrološke izide. Kljub pregledani literaturi, se še vedno poraja vprašanje, če je adrenalin učinkovit in ali bi ga bilo potrebno zamenjati s katerim drugim vazopresorjem.

Title:Use of adrenaline in cardiac arrest
Abstract:Introduction: Introduction: Cardiac arrest is a major healthcare problem, with an insignificantly rising incidence and a high number of deaths. The key to cardiac arrest is its recognition and the knowledge of healthcare workers, as rapid defibrillation and external cardiac massage are key activities to restore spontaneous circulation (ROSC). The aim of this thesis was to investigate the effects of adrenaline in cardiac arrest. Methods: A literature review on the effects of epinephrine in patients with cardiac arrest was searched in the following databases: CINAHL, ScienceDirect, PubMed and Cochrain library. All literature was searched and displayed using PRISMA diagram. Results: We were able to identify positive and negative effects of adrenaline. The most important findings showed that adrenaline in lower doses and doses with a larger time window have an effect on faster return of spontaneous circulation and favourable neurological outcome. Discussion and conclusion: We found that adrenaline, when administered quickly and in the right doses and at the right time, has a positive effect on the return of spontaneous heart rhythm and favourable neurological outcomes. Despite the literature reviewed, the question remains whether adrenaline is effective and whether it should be replaced by another vasopressor.
