Title:Uporaba brezšivne skrbi pri pacientih s pridruženimi kroničnimi nenalezljivimi boleznimi

Abstract:Uvod: Brezšivna skrb je na posameznika usmerjen proces, katere glavni namen je preverjanje skladnosti predpisane farmakološke terapije s ciljem zmanjšanja napak, ki se pojavljajo v povezavi s farmakološko terapijo. Gre za proces, ki zahteva multidisciplinarno sodelovanje strokovnjakov na različnih ravneh zdravstvenega varstva. Metode: V teoretičnem delu magistrskega dela smo uporabili deskriptivno metodo analize in sinteze virov, pridobljenih s pomočjo domače in tuje znanstvene ter strokovne literature. V empiričnem delu magistrskega dela smo izvedli intervjuje s kliničnimi farmacevti v izbrani bolnišnični instituciji, kot instrument raziskovanja pa smo uporabili pol strukturiran intervju. Rezultati: Na podlagi analize in sinteze virov ter izvedenih intervjujev smo ugotovili, da brezšivna skrb pomembno vpliva na varnost in kakovost obravnave hospitaliziranih pacientov s pridruženimi kroničnimi nenalezljivimi boleznimi in da imajo intervjuvanci dobro percepcijo glede implementacije brezšivne skrbi v prakso. Razprava in zaključek: Brezšivna skrb pomembno vpliva na zmanjševanje napak, ki se pojavljajo v povezavi s farmakološko terapijo v praksi in so pogostejše, kot si predstavljamo. Posledično lahko izpostavimo prednosti tako za posameznika kot javno-zdravstveni sistem.

Title:The use of seamless care in patients with associated chronic non-communicable diseases
Abstract:Introduction: Seamless care is an individual-focused process, the main purpose of which is to verify the compliance of prescribed pharmacological therapy with the aim of reducing errors associated with pharmacological therapy. It is a process that requires multidisciplinary collaboration of experts at various levels of healthcare. Research methods: In the theoretical part of the master's thesis, we used a descriptive method of analysis and synthesis of sources obtained through domestic and foreign scientific and professional literature. In the empirical part of the master's thesis, we conducted interviews with clinical pharmacists in the selected hospital institution, using a semi-structured interview as the research instrument. Results: Based on the analysis and synthesis of sources and conducted interviews, we found that seamless care significantly influences the safety and quality of treatment for hospitalized patients with associated chronic non-communicable diseases. The interviwee also have a positive perception regarding the implementation of semaless care into practise. Discussion and conclusion: Seamless care significantly contributes to reducing errors associated with pharmacological therapy in practise, which are more common, that we might imagine. Consequently, we can highlight the benefits for both the individual and the public healthcare system.
