Abstract:Z demografskimi spremembami in z naraščanjem obolevnosti se spreminja vloga pacienta v sodobnem zdravstvu.

Title:Model of the patient’s self-care process
Abstract:With demographic changes and increasing morbidity, the role of the patient in modern healthcare is changing. The patient’s active role is emphasized, especially in the presence of chronic non-communicable diseases, which are increasingly present among adults in the developed world, especially in the elderly population. This translates into an increasing burden on healthcare, a lower quality of life and more patients who are dependent on others for daily tasks. Therefore, it is important for the patients to be able to take care of themselves whether they are ill or in good health. The purpose of the doctoral dissertation is to design a model of the patient's self-care process and to place it into the broader context of the professional care with particular attention on the interplay and integration with other processes in healthcare. For successful implementation of the process, it is necessary to identify its key elements and to define professional support. An action science approach was used, including methods such as context derivation, context analysis, process modeling and decision-making model design. A self-care process model for a patient with chronic noncommunicable diseases has been developed. One key element of self-care process are the patient’s decisions related to health support measures. We designed a decision-making support tool based on a multi-attribute decision-making approach, which ensures the patient’s transparent and timely actions in case of symptom deterioration. The self-care process model has been placed in the patient's ecosystem in the home environment, emphasizing the provision of dynamic balance by the professionals involved. We also provided a contextual definition of the self-care concept, adapted to the Slovenian environment. Self-care of a patient with chronic non-communicable diseases is a key part of holistic approach to health care. The self-care process model for these patients has not yet been identified in the existing literature, which is why our model represents an important contribution to science. Identification of the essential elements of self-care process and inclusion of self-care into the healthcare processes will ensure a dynamic balance of the patient's ecosystem aided by the professional self-care support. The designed decision-making support tool ensures transparent decision-making. Patients with chronic noncommunicable diseases must make timely decisions about certain aspects of their health and decide in time to seek professional help, which can contribute to their optimal health and wellness. Our definition of the self-care concept should encourage Slovenian researchers to explore more of the elements of patient self-care and to design effective professional interventions. The self-care process with all its essential elements, which can be supported by digital technology, must become an integral part of the healthcare system.
