Title:Merjenje povprečnega sistemskega tlaka polnjenja srca v enoti intenzivne terapije

Abstract:Uvod: Medicinskim sestram in zdravnikom so metode povprečnega sistemskega tlaka polnjenja srca zelo slabo poznane in nerazumljive. V raziskavi smo predstavili metode, ki jih priporočajo avtorji, in prikazali možnost izvajanja meritev teh metod. Metodologija: Meritve smo opravili v kliničnem okolju, na oddelku za intenzivno terapijo, nego, anestezijo in terapijo bolečine v splošni bolnišnici dr. Jožeta Potrča Ptuj. Opravili smo jih s pomočjo monitoringa, ki vključuje obstoječe invazivno merjenje arterijskega tlaka, merjenje centralnega venskega tlaka in manšeto z možnostjo zaustavitve krvnega pretoka v roki s funkcijo za kontinuirano napihnjenost (Venous stasis). Potrebovali smo tudi ventilator za umetno ventilacijo in hemodinamski monitoring. Rezultati: S pregledom podatkov strokovno znanstvene literature smo predstavili metode za oceno Pmsf pri živalih in obstoječe metode meritev pri ljudeh, ki jih priporočajo avtorji. Izvedli smo meritve Pmsf z metodo zaustavitve obtoka v roki (Parm), kjer je po hitrem napihovanju manšete arterijski tlak uravnotežen po 25–30 sekundah, in takrat je moč odčitati vrednost Pmsf. Prikazali smo si COmf in CVP ter predvidevali, da bi si z uporabo metode zadrževanja vdiha lahko z grafičnim izrisom prikazali razmerje med meritvama in s tem pridobili vrednost Pmsf. Diskusija: Ugotavljali smo, ali so meritve Pmsf izvedljive, in medicinskim sestram pokazali možnost prikaza določenih metod v kliničnem okolju. Sklep: V klinični praksi ugotavljamo, da je ocena hemodinamskega stanja pomembna za potek obravnave in zdravljenje bolnika. Ker so meritve Pmsf v slovenskem okolju še vedno praktično neznane, bi bila izobraževanja iz te tematike zelo dobrodošla.

Title:Measurement of mean systemic filling pressure in the intensive care unit
Abstract:Introduction: Nurses and doctors do not know and comprehend a lot about the methods of mean systemic filling pressure. Therefore, the literature has been studied and the methods which are recommended by authors are presented. Moreover, the possibilities of measurement of aforementioned methods are given. Methods: The measurements were made in clinical setting at the Intensive Care Unit at General Hospital dr. Jože Potrč Ptuj. The measurements were made through means of monitoring, which includes existing invasive measurements of arterial blood pressure, measurement of central venous pressure, measurements with inflatable cuff where blood flow cessation is manual and has the function of continued inflammation (Venous stasis). Moreover, the ventilator for artificial ventilation and hemodynamic monitoring was applied. Results: The data from technical and scientific literature was checked and the methods for the assessment of Pmsf in animals were presented; moreover, the existing recommended methods for measurements in human were presented. The Pmsf measurements were implemented with the method of manual blood flow cessation (Parm), where the arterial pressure is balanced after 25 to 30 seconds, if the cuff is quickly inflated; after 25 to 30 seconds, the level of Pmsf can be measured. The COmf and CVP were shown; it was foreseen that by the use of breath holding method the rate between the measurements could be shown by graphic scan and thus the Pmsf levels could be obtained. Discussion: It was established whereas the Pmsf measurements are practicable. Moreover, the presentation of specific methods in clinical setting was shown to the nurses.
