Title:ORGANIZACIJSKO INFORMACIJSKI MODEL PREPREČEVANJA NAPAK V PROCESU ZDRAVSTVENE NEGE

Abstract:Varnost pacientov je temeljnega pomena za kakovost zdravstvenega varstva in s tem tudi zdravstvene nege, ki temelji na preventivnih pristopih in sistematičnih analizah poročil pacientov in zdravstvenega osebja o varnostnih zapletih. Vsak korak v oskrbi pacienta vključuje možnosti za napake in določeno stopnjo tveganja za pacientovo varnost. V disertaciji smo najprej predstavili proces zdravstvene obravnave in zdravstvene nege ter opredelili dejavnike, ki vplivajo na kritično razmišljanje in klinično odločanje medicinskih sester, s tem pa tudi na varnost pacienta. V nadaljevanju smo predstavili nekatere metode za ocenitev tveganj za nastanek napak v procesu in jih med seboj primerjali. Osrednji prispevek doktorske disertacije predstavlja organizacijsko informacijski model preprečevanja napak v procesu zdravljenja in zdravstvene nege. Za analizo delovnih procesov smo uporabili Metodo strukturirane analize. Za ocenitev tveganja za nastanek napak v procesu zdravljenja smo uporabili Metodo analize možnih napak in njihovih posledic prilagojeno za zdravstvo. Za opredelitev dejavnikov, ki vplivajo na nastanek napak smo uporabili Metodo analize temeljnih vzrokov. V nadaljevanju smo predstavili razvoj in oceno prototipnega modela elektronsko podprtega procesa zdravljenja in zdravstvene nege s poudarkom na predhodno analizo tveganj za nastanek napak. Gre za objektno usmerjen model, ki omogoča celovit dostop do vseh elektronskih zdravstvenih zapisov pacienta. Omogoča transparentnost procesa, enostavno validacijo in verifikacijo posameznih faz v procesu zdravljenja in zdravstvene nege, ter vzpodbuja kritično razmišljanje zdravstvenih izvajalcev in boljše odločanje. Prototip modela smo testirali v kliničnem okolju. Na podlagi analize prenovljenega procesa ugotavljamo, da je nov organizacijsko informacijski model pomembno vplival na komunikacijo in informiranje, kritično razmišljanje, izkušnje in znanje, organizacijo dela in kompleksnosti delovnih nalog, s tem pa na strokovno pozornost izvajalcev zdravstvene obravnave in zmanjšanje napak. Model predstavlja orodje za zmanjševanje tveganj za nastanek napak v procesu zdravstvene nege. Metode, ki smo jih uporabili pri razvoju modela so uporabne za posamezne segmente v procesu zdravljenja in zdravstvene nege. Pričajoče raziskovalno delo odpira številne možnosti za nadaljnje raziskave na področju načrtovanja kliničnih poti, določanja negovalnih diagnoz in intervencij ter predpisovanja zdravil. Potrebno je izgraditi ekspertne sisteme, ki bi na osnovi skrbno določenih protokolov predlagali klinično pot, negovalno diagnozo oz terapijo ter opozarjal na morebitna odstopanja.

Title:ORGANIZATIONAL INFORMATION MODEL FOR PREVENTING ERRORS IN THE PROCESS OF NURSING
Abstract:Patient safety is fundamental to the quality of nursing and health care. Patient safety is based on preventative approaches and systematic analysis of the reports of patients and medical personnel on security incidents. Each step in the care of a patient includes the possibility of error and a degree of risk to patient safety. In the first part of the thesis, we presented the process of nursing and health care. We have identified factors that affect the critical thinking and clinical decision-making of nurses and consequently on patient safety. Below we present some methods for assessing the risk of errors in the process and compare them with each other. The main contribution of the doctoral thesis presents organizational information model for preventing errors in nursing and health care. For the analysis of work processes, we used the method of structured analysis. In order to assess the risk of errors in the process, we used the Healthcare Failure Mode and Effect Analysis. To identify factors that influence the formation of defects, we used the Root Cause Analysis. Below, we present the development and evaluation of a prototype model of electronic-based process health care with an emphasis on a preliminary analysis of the risk of errors. It is an object-oriented model, which allows full access to all electronic healthcare records patient. It enables process transparency, easy validation and verification of the various stages in the process of nursing and health care. The model encourages health care providers of critical thinking and improves decision-making. A prototype model was tested in a clinical setting. Based on analysis of the revised process, we find that the new organizational information model had a significant impact on communication and information, critical thinking, experience and knowledge, work organization and complexity of tasks. This affects on healthcare profession attention and reduces errors. The model is a tool for reducing the impact of factors that give rise to errors in the process of nursing care. The methods we have used in the development of the model are useful for individual segments in the process of treatment and health care. Presented research work opens up numerous possibilities for further research into the design of clinical pathways, identifying nursing diagnoses and interventions and prescription medications. It is necessary to build expert systems that would be closely based on certain protocols proposed clinical pathways, diagnosis or therapy with attention to any discrepancies.
