Title:Enodnevna obravnava bolnika pri operaciji dimeljske kile v lokalni anesteziji in pomen informiranja širše javnosti

Abstract:Operacije kile sodijo med najpogostejše trebušne operacije, saj ima to težavo skoraj vsak osmi moški. Kile je mogoče operirati v lokalni, regionalni ali splošni anesteziji. Lokalna anestezija pomeni bistveno nižji strošek kot splošna in omogoča stik z bolnikom med operacijo. Nedvomno pa je mogoče tudi v Sloveniji z racionalnejšim ravnanjem na nacionalni ravni zmanjšati celotne stroške zdravljenja dimeljskih kil. Uporabljati je potrebno sodobne operativne metode, ker so tako stroški ozdravitve kile manjši. Uporabili smo deskriptivno metodo dela. Kot instrument raziskave pa smo uporabili anketni vprašalnik. Za obdelavo podatkov, ki smo jih pridobili z anketnim vprašalnikom, smo uporabili statistično metodo frekvenčne porazdelitve posameznih odgovorov na celotni vzorec. Rezultate smo predstavili v odstotkih in jih ponazorili z grafi. Namen raziskave je bil ugotoviti zadovoljstvo bolnikov z enodnevno obravnavo, zakaj so se odločili za poseg v lokalni anesteziji, njihove težave po odpustu iz Dnevne bolnišnice, mnenje o telefonskem klicu medicinske sestre pred in po posegu in ugotoviti motiviranost bolnikov po dodatnem izobraževanju pred operacijo. Ugotovili smo, da so bolniki z enodnevno obravnavo zadovoljni, saj bi se jih 82 % ponovno odločilo za poseg v lokalni anesteziji. Glavni razlog odločitve za ta poseg je enodnevna hospitalizacija. Največjo težavo bolniki prvi dan po odpustu iz bolnišnice navajajo bolečino. Njihovo zadovoljstvo potrjujeta telefonska klica medicinske sestre pred in po posegu. Ugotovili smo 55 % motiviranost anketiranih bolnikov po dodatnem izobraževanju pred operacijo. Po oceni bolnikov ugotavljamo dobro informiranost pred operativnim posegom, njihovo zadovoljstvo s poučevanjem na oddelku Dnevne bolnišnice ter s telefonskim pogovorom z medicinsko sestro- koordinatorko zdravljenja pred in po posegu.

Title:Day care inguinal hernia surgery under local anaesthesia and the significance of informing the general public
Abstract:Hernia surgeries are some of the most common abdominal surgeries, with nearly one in eight men suffering from this disorder. Hernia repairs can be performed under local, regional or general anaesthesia. Local anaesthesia is connected with significantly lower costs than general anaesthesia, while at the same time allowing contact with the patient during the surgery. By employing cost-effective methods at the national level it is undoubtedly possible to decrease the overall costs of inguinal hernia treatment in Slovenia. Therefore, advanced surgical methods incurring lower treatment costs should be applied. We used descriptive method. As a research instrument, we used a questionnaire. The processing data acquired by the questionnaire, we used a statistical method of frequency distribution of individual responses to whole sample. The results were presented in percentages and are represented by graphs. The research objective was to determine the patients' satisfaction with the day care; why they decided to undergo a surgical intervention under local anaesthesia; which problems they experienced after release from the Day Hospital; what their opinion was about the telephone calls received from the nurse before and after the intervention, respectively; and how motivated they were for receiving additional instructions before the surgery. We established that the patients were satisfied with the day care, with 82% responding they would again decide on surgery under local anaesthesia. The main reason for this decision is the one-day hospitalisation. The most prevalent problem experienced by the patients on the day following their release from hospital was pain. The patients' satisfaction was confirmed by two telephone calls made by the nurse before and after the surgery, respectively. We further established that 55% of the patients were interested in additional instructions before the surgery. According to the patients' assessment, we find that they are well informed before the surgery. They are satisfied with the information and instructions received at the Day Hospital as well as with the telephone conversation with the nurse acting as the treatment co-ordinator before and after the surgery, respectively.
