Evaluation of arterial hypertension control in family practice in Slovenia

Abstract: Objective: We performed a study on almost 20,000 Slovene patients with arterial hypertension (AH) to evaluate age- and gender-dependent blood pressure control, differences in the rate of AH control in the period 2002-2008, and to validate a potential impact of selected quality indicators on blood pressure control. Methods: The study was conducted as a part of the "Quality of Healthcare in Slovenia" project, in agreement with the National Medical Ethics Committee of the Republic of Slovenia. Appropriate statistical analyses were performed and the results evaluated. Results: Arterial hypertension control was relatively high (55.8%, 95% CI: 55.1-56.5) in the period 2002-2008 and improved significantly during that period. Based on our statistical model, the improved AH control in year 2006 compared to 2002 is particularly due to lower initial blood pressure values before treatment. Uncontrolled AH was largely attributed to uncontrolled systolic blood pressure. We found positive association between AH control and the frequency of blood pressure control in less than six-month time intervals. Conclusions: According to our results, AH control in family practice in Slovenia is relatively high compared to other European countries, but the results refer only to patients visiting their family medicine physicians.

Prognostic significance of uPA/PAI-1 level, HER2 status, and traditional histologic factors for survival in node-negative breast cancer patients

Abstract: Background: The association of HER2 status with urokinase plasminogen activator (uPA) and plasminogen activator inhibitor 1 (PAI-1) levels raises the question whether uPA/PAI-1 level carries additional clinically relevant prognostic information independently from HER2 status. The aim of our study was to compare the prognostic value of uPA/PAI-1 level, HER2 status, and traditional prognostic factors for survival in node-negative breast cancer patients. Patients and methods: A retrospective analysis of 858 node-negative breast cancer patients treated in Maribor University Clinical Center, Slovenia, in the years 2000-2009 was performed. Data were obtained from patient medical records. The median follow-up time was 100 months. Univariate and multivariate analyses of disease-free (DFS) and overall survival (OS) were performed using the Cox regression and the Cox proportional hazards model. Results: In univariate analysis, age, tumor size, grade, lymphovascular invasion, HER2 status and UPA/PAI-1 level were associated with DFS, and age, tumor size, grade, and uPA/PAI-1 level were associated with OS. In the multivariate model, the most important determinants of DFS were age, estrogen receptor status and uPA/PAI-1 level, and the most important factors for OS were patient age and tumor grade. The HR for death from any cause in the multivariate model was 1.98 (95% CI 0.83-4.76) for patients with high uPA and/or PAI-1 compared to patients with both values low. Conclusions: uPA/PAI-1 level clearly carries an independent prognostic value regardless of HER2 status in node-negative breast cancer and could be used in addition to HER2 and other markers to guide clinical decisions in this setting.

The effect of an educational intervention in family phisicians on self-rated quality of life in patients with medically unexplained symptoms

Abstract: Introduction: Medically unexplained symptoms (MUS) are very common in family medicine, despite being a poorly-defined clinical entity. This study aimed to evaluate the effect of an educational intervention (EI) on self-rated quality of life, treatment satisfaction, and the family physician-patient relationship in patients with MUS. Methods: In a multi-centre longitudinal intervention study, which was performed between 2012 and 2014, patients were asked to rate their quality of life, assess their depression, anxiety, stress and somatisation, complete the Hypochondriasis Index, the Medical Interview Satisfaction Scale and the Patient Enablement Instrument for assessing the physician-patient relationship, before and after the EI. Results: The mean values before and after the intervention showed that after the EI, patients with MUS gave a lower (total) mean rating of their health issues and a higher rating of their quality of life, and they also had a more positive opinion of their relationship with the physician (p<0.05). However, there were no differences in the (total) rating of treatment satisfaction before and after the EI (p=0.423). Significant differences in the symptoms in patients with MUS before and after the intervention were confirmed for stress, somatisation and hypochondriasis (p<0.05). Conclusions: It could be beneficial to equip family physicians with the knowledge, skills and tools to reduce hypochondriasis and somatisation in MUS patients, which would improve patients’ self-rated health status.

The influence of electrogalvanic device on scaling

Abstract: The use of an electrogalvanic device for scale neutralisation is descibed in this paper. Physico-chemical analyses were performed before and after the treatment. The results were compared with those obtained by using magnetic water treatment device. By measuring some individual parameters and the implementation of chemical analysis, the satisfactory functioning of the electrogalvanic device was demonstrated. The quality of drinking water did notchange much after the water treatment method. The results of determination of calcium carbonate saturating index showed that the raw drinking water is incarbonate equlibrium as well as both treated water samples. The calcite/aragonite ratio was studied by means of microscopy and X-ray powder diffraction. Inspection of crystals formed during the experiments with microscopy indicated that aragonite crystal structure of the precipitates prevailed over the calcite stucture. The diffractograms showed that the share of aragonite increased after using the electrogalvanic device compared with raw drinking water samples where the share of calcite was higher.

Zastrupitev z etilenglikolom

Abstract: Zastrupitve z etilenglikolom so dokaj redke. Etilenglikol je sam po sebi malo toksičen, hudo zastrupitev povzročajo predvsem njegovi presnovni produkti. Prikazani primer obravnava značilno klinično sliko (nevrološka, kardiopulmonalna in ledvična faza) in laboratorijske rezultate (povišana serumska osmolalnost, povišana osmolalna vrzel, metabolična acidoza s povišano anionsko vrzeljo, kristali kalcijevega oksalata v urinu) ob zastrupitvi z etilenglikolom. Hitra prepoznava je nujna, saj sta obolevnost in smrtnost po zastrupitvi visoki. Zdravljenje vključuje vzdrževanje dihalne poti, dihanja in krvnega obtoka, infuzijo bikarbonata, dajanje antidota (etanol, fomepizol), hemodializo, včasih tudi zdravljenje s kofaktorji presnove etilenglikola (tiamin, piridoksin, magnezij).

The influence of physico-chemical parameters on water scale precipitation on washing machines' heaters

Abstract: The paper presents the results of analysis of physico - chemical parameters influence on the intensity of water scale precipitation on washing machines heaters. Washing machines of the same producer were, to this end, modified to an endless cycle of washing with the same amount of cotton fabric at the same conditions. On the basis of various experiments it was determined that water hardness, the amount of exceeded carbon dioxide, water turbulence around the heater, the amount of cotton fibres in water exceeded from fabrics, and heater specific heat strength have the key role in water scale building up in washing machines heaters.

Prenova bolnišničnega informacijskega sistema “IS VZE-MEDIC”

Abstract: Tema diplomskega dela je prenova bolnišničnega informacijskega sistema z nazivom IS VZE MEDIC Vojaško zdravstvene enote. V prvem delu je opisan sistem in predstavljene specifičnosti povezovanja z ostalimi informacijskimi sistemi znotraj zaprtega intranetnega omrežja. V drugem delu je opisana interoperabilnost bolnišničnega informacijskega sistema z različnimi sistemi v zvezi NATO za povezovanje doma in v tujini. Preučena je bila različna domača in tuja literatura, dokumenti organizacije in predpisani standardi zveze NATO (STANAG – angleško Standardization Agreement).

InVance sling postopek za zdravljenje moške stresne urinske inkontinence

Abstract: Namen: Stresna inkontinenca urina je pri moških relativno redka in se pojavi kot posledica večjega operativnega posega, drugi vzroki stresne inkontinence urina se pri moških pojavljajo izjemoma. Ne glede na relativno majhno število prizadetih takšno uhajanje urina obolelim predstavlja velik medicinski in tudi socialni problem, saj se odpovejo normalnim dnevnim aktivnostim in se vse bolj omejujejo na domače okolje. Metode: Ena izmed možnosti zdravljenja stresne urinske inkontinence pri moških je tako imenovani InVance Male Sling sistem. Gre za minimalno invazivno kirurško metodo, pri kateri se na spodnjo stran pubične kosti z vijaki pritrdi poliesterska mrežica, ki tesni sečnico. V principu gre za sling operacijo. Rezultati: Na našem oddelku smo med letoma 2004 in 2007 z omenjeno prometodo zdravili osemintrideset bolnikov. Pri štiriintridesetih je šlo za stanje po radikalni odstranitvi prostate, pri štirih pa za stanje po transuretralni resekciji prostate. Čas spremljanja po posegu je pri vseh daljši od dveh let. Osemindvajset bolnikov navaja popolno ozdravitev (brez uporabe predlog), šest bolnikov navaja blago stresno inkontinenco (ena predloga dnevno), pri štirih bolnikih pa ni prišlo do izboljšanja. Pri nobenem bolniku nismo opažali zastoja urina ali na novo nastalih težav pri uriniranju, smo pa pri treh bolnikih srečali podaljšano gnojno izcejanje iz postoperativne rane in še pri treh kasno zavrnitev mrežice, zato smo pri teh bolnikih morali mrežico odstraniti. Imeli smo tudi bolnika z erozijo sečnice osem mesecev po posegu. Zaključek: Glede na naše rezultate lahko zapišemo, da je Invance sling operacija uspešna metoda zdravljenja moške stresne inkontinence urina z minimalnimi pooperativnimi zapleti.
