Hemodynamic response to high- and low-load resistance exercise in patients with coronary artery disease : a randomized, crossover clinical trial

Abstract: Low-load resistance exercise (LL-RE) is recommended as an adjunct therapy to aerobic exercise during cardiac rehabilitation in patients with coronary artery disease. The safety and hemodynamic response to high-load (HL) RE remain unknown. The aim of this study was to evaluate the hemodynamic response during both HL-RE and LL-RE prior to cardiac rehabilitation. Forty-three patients with coronary artery disease and/or percutaneous coronary intervention performed three sets of leg-press exercise using HL-RE (eight repetitions at the intensity of 80% of one repetition maximum (1-RM)) and LL-RE (16 repetitions at the intensity of 40% 1-RM) in a randomized crossover sequence. Heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP), and rating of perceived exertion were measured at baseline, after each set of RE and post-exercise. No clinically relevant changes in HR and BP or in patient-reported symptoms were recorded during HL-RE or LL-RE. Compared with baseline, HR and SBP increased during LL-RE (from 66 bpm to 86 bpm, time effect: p < 0.001; from 129 mmHg to 146 mmHg, time effect: p < 0.001) and HL-RE (from 68 bpm to 86 bpm, time effect: p < 0.001; from 130 mmHg to 146 mmHg, time effect: p < 0.001). Compared with HL-RE, the increase in HR was greater after the final set of LL-RE (32% vs. 28%, p = 0.015), without significant differences in SBP and DBP between LL-RE and HL-RE. Rating of perceived exertion was higher after the 1st set of HL-RE compared with LL-RE (median (interquartile range): 6 (5–7) vs. 6 (5–6), p = 0.010). In patients with coronary artery disease, both HL-RE and LL-RE were safe and well-tolerated. Hemodynamic changes were similar and within the physiological response to RE.

Assessment of kidney function : clinical indications for measured GFR

Abstract: In the vast majority of cases, glomerular filtration rate (GFR) is estimated using serum creatinine, which is highly influenced by age, sex, muscle mass, body composition, severe chronic illness and many other factors. This often leads to misclassification of patients or potentially puts patients at risk for inappropriate clinical decisions. Possible solutions are the use of cystatin C as an alternative endogenous marker or performing direct measurement of GFR using an exogenous marker such as iohexol. The purpose of this review is to highlight clinical scenarios and conditions such as extreme body composition, Black race, disagreement between creatinine- and cystatin C–based estimated GFR (eGFR), drug dosing, liver cirrhosis, advanced chronic kidney disease and the transition to kidney replacement therapy, non-kidney solid organ transplant recipients and living kidney donors where creatinine-based GFR estimation may be invalid. In contrast to the majority of literature on measured GFR (mGFR), this review does not include aspects of mGFR for research or public health settings but aims to reach practicing clinicians and raise their understanding of the substantial limitations of creatinine. While including cystatin C as a renal biomarker in GFR estimating equations has been shown to increase the accuracy of the GFR estimate, there are also limitations to eGFR based on cystatin C alone or the combination of creatinine and cystatin C in the clinical scenarios described above that can be overcome by measuring GFR with an exogenous marker. We acknowledge that mGFR is not readily available in many centres but hope that this review will highlight and promote the expansion of kidney function diagnostics using standardized mGFR procedures as an important milestone towards more accurate and personalized medicine.

Impact of dual dispatch system implementation on response times and survival outcomes in out-of-hospital cardiac arrest in rural areas

Abstract: Objectives: Dual dispatch early defibrillation in out-of-hospital cardiac arrest (OHCA) victims provided by firefighters in addition to Emergency medical services (EMS) has proven to increase rate of return of spontaneous circulation (ROSC) and thus survival in the metropolitan or suburban areas whereas the data in rural areas are scarce. Methods: This was a retrospective observational cohort study of EMS resuscitated OHCA victims in regions with dual dispatch of volunteer firefighters as first responders (intervention group). Historical group was based on all OHCAs occurring in these regions before the implementation of first responders (EMS response only). Multivariate logistic regression with following variables: intervention, age, gender, witnessed status, bystander cardiopulmonary resuscitation (CPR), first rhythm and etiology were used to control for confounding factors affecting ROSC. Results: A total of 312 OHCAs were included in the study (historical group, n = 115 and intervention group, n = 197). Median time to arrival of first help shortened significantly for all patients, patients with ROSC and patients with Cerebral Performance Category 1/2 (CPC 1/2) in intervention vs historical group (8 vs 12 min, p < 0.001; 7.5 vs 11 min, p = 0.002; 7 vs 10 min, p = 0.011; respectively). The proportion of patients with ROSC, 30-day survival and CPC 1/2 at hospital discharge remained unchanged in intervention vs historical group (21% vs 23%, p = 0.808; 7% vs 6%, p = 0.914; 6% vs 3%, p = 0.442; respectively). The logistic regression model of adjustment confirms the absence of improvement in the ROSC rate after the implementation of first responders. Conclusions: Introduction of a dual dispatch of local first responders in addition to EMS in cases of OHCA significantly shortened response times. However, reduced response times were not associated with better survival outcomes.

The Impact of Cardiorespiratory and Metabolic Parameters on Match Running Performance (MRP) in National-Level Football Players : a multiple regression analysis

Abstract: The aim of the study was to examine the association between cardiorespiratory and metabolic parameters and match running performance (MRP) in highly trained football players. The sample of participants consisted of 41 national-level football players (aged 23.20 ± 3.40 years, body height 182.00 ± 5.15 cm, and body mass 76.86 ± 6.06 kg) from the Serbian Super league. For the purposes of this research, the following measurements were applied. A maximal multistage progressive treadmill test, with a direct measurement of maximal oxygen consumption (VO2max) (using Fitmate MED, Cosmed, Rome, Italy) was conducted, alongside continuous heart rate monitoring. Capillary blood samples were taken from the hyperemic area using specific test strips, and, after sample collection, lactate concentration was immediately determined using a lactate analyzer. MRP variables were analyzed according to the BioIRC model of motion structure analysis, based on existing standards for profiling movement intensity. The results of multiple regression analysis indicated an association between cardiac parameters and total distance (R2 = 54.3%, p = 0.000), high-speed running (R2 = 46.4%, p = 0.000), and jogging (R2 = 33.6%, p = 0.004). Regression analysis revealed an association between cardiorespiratory parameters and total distance (R2 = 24.8%, p = 0.014), and high-speed running (R2 = 20%, p = 0.039). Meanwhile, no association was found between lactate concentration and running performance. The explanation for these regression analysis results is based on the observation that functional abilities represent significant potential for expressing movement performance, a crucial condition for success in football.

High strain rate hardening of metallic cellular metamaterials

Abstract: Strain rate hardening caused by the changed deformation mode is a fascinating phenomenon in cellular metamaterials where the material’s stiffness and energy absorption capabilities increase as the strain rate increases. This unique behaviour is attributed to a combination of micro-inertia effects, base material’s strain rate hardening and inertia effects. At high strain rates, the metamaterial’s inertia influences its deformation response, which changes to shock mode. This work briefly presents the geometry and fabrication of different metallic metamaterials. Then, it evaluates their mechanical response at different strain rates, ranging from quasi-static to intermediate dynamic and shock, determined by experimental and computational investigation. The three deformation modes can be separated into two critical loading velocities, unique for each metamaterial, which are also presented and compared in this work for various metamaterials. The investigations show that the deformation mode change in metallic metamaterials depends on their porosity. The critical velocities separating the deformation modes decrease with increasing porosity, i.e., decreased density of the metamaterial results in reduced critical loading velocities. The shock deformation mode in cellular metamaterials is thus attainable at much lower loading velocities than in homogeneous (nonporous) materials.

Mechanical behaviour of photopolymer cell-size graded triply periodic minimal surface structures at different deformation rates

Abstract: This study investigates how varying cell size affects the mechanical behaviour of photopolymer Triply Periodic Minimal Surfaces (TPMS) under different deformation rates. Diamond, Gyroid, and Primitive TPMS structures with spatially graded cell sizes were tested. Quasi-static experiments measured boundary forces, representing material behaviour, inertia, and deformation mechanisms. Separate studies explored the base material’s behaviour and its response to strain rate, revealing a strength increase with rising strain rate. Ten compression tests identified a critical strain rate of 0.7 s−1 for “Grey Pro” material, indicating a shift in failure susceptibility. X-ray tomography, camera recording, and image correlation techniques observed cell connectivity and non-uniform deformation in TPMS structures. Regions exceeding the critical rate fractured earlier. In Primitive structures, stiffness differences caused collapse after densification of smaller cells at lower rates. The study found increasing collapse initiation stress, plateau stress, densification strain, and specific energy absorption with higher deformation rates below the critical rate for all TPMS structures. However, cell-size graded Primitive structures showed a significant reduction in plateau and specific energy absorption at a 500 mm/min rate.

Comparison of the mechanical properties of hardfacings made by standard coated stick electrodes and a newly developed rectangular stick electrode

Abstract: Cladding with a stick electrode is one of the oldest arc processes for adding a deposit on a base material. The process is suitable for outdoor working, but the disadvantages are low productivity and large dilution rates. In this work, a simple solution is proposed, which would enable cladding of a larger area with one pass and decrease the dilution rate at the same time—a new type of electrode was developed, exhibiting a rectangular cross-section instead of a round one. Hardfacings, welded with E Fe8 electrodes according to EN 14 700 Standard were welded on mild steel S355 J2 base material with three different coated stick electrodes. The first one was a commercially available, standard, round hardfacing electrode, the second was the same, but with a thinner coating, and the third one was a newly developed rectangular electrode. All three types had equal cross-sections of the metallic core and the same type of coating. Manufacturing of the rectangular electrodes in the laboratory is explained briefly. One- and multi-layer deposits were welded with all three types. Differences were observed in the arc behavior between the round and rectangular electrodes. With the rectangular electrode, the microstructure of the deposit was finer, penetration was shallower, and dilution rates were lower, while the hardness was higher, residual stresses predominantly compressive, and the results of instrumented Charpy impact tests and fracture mechanics tests were better.

Electrodermal activity and heart rate variability for detection of peripheral abnormalities in type 2diabetes : a review

Abstract: Modern medicine exhibits an upward trend towards non-invasive methods for early detection of disease and long-term monitoring of patients’ health. Diabetes mellitus and its complications are a promising area for implementation of new medical diagnostic devices. One of the most serious complications of diabetes is diabetic foot ulcer. The main causes responsible for diabetic foot ulcer are ischemia caused by peripheral artery disease and diabetic neuropathy caused by polyol pathway-induced oxidative stress. Autonomic neuropathy impairs function of sweat glands, which can be measured by electrodermal activity. On the other hand, autonomic neuropathy leads to changes in heart rate variability, which is used to assess autonomic regulation of the sinoatrial node. Both methods are enough sensitive to detect pathological changes caused by autonomic neuropathy and are promising screening methods for early diagnosis of diabetic neuropathy, which could prevent the onset of diabetic ulcer.

Drivers and barriers for the adoption of circular economy principles towards efficient resource utilisation

Abstract: Contemporary societies, whether they have reached advanced levels of economic and social development or are still developing, need to prioritise the implementation of circular economy practices. This will facilitate the shift towards regenerative, sustainable, and closed-loop industrial systems. For now, there are some efforts to encourage patterns of production and consumption through changes in economic trends, development of institutional frameworks, harmonising regulations in the field of environmental protection, and raising the level of social awareness to achieve, above all, greater efficiency of resources. Extensive literature that deals with monitoring the implementation of the circular economy already indicates the existence of numerous barriers in this process but also notices some incentives for raising the degree of circularity of the observed systems. In this paper, the question of whether a single value can represent the level of circularity of the chosen material/product chain is researched. An overview of barriers and drivers for implementing circular economy strategies is given to structure a proper metric framework in correlation to the research question. The objective was to simplify the monitoring of circularity by developing a unique index for comparing material/product chains in similar systems. This was accomplished by using the circular material use (CMU) rate introduced in the European Union and modifying it for financial data calculation. As a result, the circular economy index, which covers all 9R strategies (CEIR), is obtained. The practical verification of model applicability was shown by determining the degree of circularity achieved for passenger cars in the four observed EU countries.
