Knjiga, film in igra: Adaptacijska analiza Heart of Darkness, Apocalypse Now in Spec Ops: The Line

Opis: Magistrsko delo se ukvarja z romanom Heart of Darkness ter njegovima priredbama. Delo je razdeljeno na dva dela. V prvem sestavimo teoretično ogrodje s katerim lahko opravimo analize, ki sledijo v drugem delu naloge. Vendar tega ogrodja ne gre enakomerno uporabljati zaradi značilnosti posameznih medijev, torej knjige, filma in računalniške igre. Tako so ugotovitve o gibanju kamere kot tudi o podobnih elementih omejene zgolj na vizualna medija, saj se v pisni obliki ne morejo pojaviti. Analitičen del naloge je razdeljen v tri poglavja, ki dela obravnavajo tako v zaporedju izdaj, kot v zaporedju, ki omogoča, da lahko vsako poglavje osnuje svojo analizo na ugotovitvah oz. podatkih prejšnjih poglavij. Tako moramo prvo analizirati knjigo na kateri osnujemo naš pregled filmske priredbe, ki nato služi sočasno s knjigo kot temelj na katerem lahko analiziramo računalniško igro. Primerjalne ugotovitve analiz se nato še enkrat pojavijo v sklepu.

Techniques of measuring heart rate in cattle

Opis: The intensive animal breeding systems can cause severe environment degradation. However, the highly productive animals are extremely sensitive to the environmental changes. The animal welfare can be in direct contradiction with the need for highly intensive and economically successful milk production. The research is aimed at complementing the method of establishing the stress in calves, race heifers, fattened cattle and milk-cows with different body masses. Measuring of heart rate is one of painless measurements of physiological parameters of stress and, from the point of view of animals it has many advantages over the measurements including taking of blood samples. For measuring the heart rate of different categories of cattle without interference into their body we have tested the usefulness of special apparatuses used worldwide by top sportsmen during training for following up, measuring and monitoring the heart rate. These are Polar monitors of the heart rate, made by Finnish company Polar Electro Oy and working wirelessly with ECG precision. The receiver counts the heart rate on the basis of pulse-to-pulse time average algorithms in 5, 15 or 60-second intervals. The computer interface ensures magnetic transfer of measured and saved data on animals into a PC where the analysis of each collected data file follows. The used type of transmitter "Polar sport tester profi" having two electrodes in an air-tight sealed frame has enabled us to obtain a good signal on calves of up to 180 kg body mass. Periodic response in case of young cattle of up to 280 kg body mass has been registered only if electrolyte was added between the animal skin and the feeler. By further modifications and tests of the mentioned apparatuses the expected response has been reached also on adult animals. We found that use of these apparatuses on animals did not have any influence on special animal acting behaviour. The production of milk and meat in herds did not change during the test.

Comorbidities and characteristics of coronary heart disease patients: their impact on health-related quality of life

Opis: Background: Patients with coronary heart disease (CHD) commonly present with more than one comorbid condition, contributing to poorer health-related quality of life (HRQoL). The aim of our study was to identify the associations between HRQoL and patient characteristics, vascular comorbidities and anxiety/depression disorders. Methods: This observational study was conducted in 36 family medicine practices selected by random stratified sampling from all regions of Slovenia. HRQoL was assessed using the European Quality of Life - 5 Dimensions (EQ-5D) questionnaire and EQ Visual Analogue Scale (EQ-VAS). The associations between HRQoL and patient characteristics stratified by demographics, vascular comorbidities, health services used, their assessment of chronic illness care, and anxiety/depression disorders were identified by ordinal logistic regression and linear regression models. Results: The final sample included 423 CHD patients with a mean age of 68.0 ± SD 10.8 years; 35.2% were female. Mean EQ-VAS score was 58.6 ± SD 19.9 (median: 60 with interquartile range of 45–75), and mean EQ-5D index was 0.60 ± SD 0.19 (median: 0.56 with interquartile range of 0.41–0.76). The statistically significant predictors of a lower EQ-VAS score were higher family physician visit frequency, heart failure (HF) and anxiety/depression disorders (R² 0.240; F = 17.368; p < 0.001). The statistically significant predictor of better HRQoL, according to EQ-5D was higher patient education, whereas higher family physician visit frequency, HF and peripheral artery disease (PAD) were predictors of poorer HRQoL (Nagelkerke R 2 = 0.298; χ 2 = 148.151; p < 0.001). Conclusions: Results of our study reveal that comorbid conditions (HF and PAD), family physician visit frequency and years in education are significant predictors of HRQoL in Slovenian CHD patients.

Combination of lung ultrasound (a comet-tail sign) and N-terminal pro-brain natriuretic peptide in differentiating acute heart failure from chronic obstructive pulmonary disease and asthma as cause of acute dyspnea in prehospital emergency setting

Opis: Introduction: We studied the diagnostic accuracy of bedside lung ultrasound (the presence of a comet tail sign), N-terminal pro-brain natriuretic peptide (NT-proBNP), and clinical assessment (modified Boston criteria) in differentiating heart failure (HF)- related acute dyspnea from pulmonary (COPD/asthma) related acute dyspnea in the prehospital setting. Methods: Prospective study was performed at the Center for Emergency Medicine Maribor, Slovenia, between July 2007 and April 2010. Two groups of patients were compared: HF-related acute dyspnea group (n = 129) vs pulmonary-related (asthma/COPD) acute dyspnea group (n = 89). All patients underwent lung ultrasound examination, along with basic laboratory, rapid NT-proBNP testing and chest X-ray. Results: Ultrasound comet tail sign has 100% sensitivity, 95% specificity, 100% negative predictive value (NPV) and 96% positive predictive value (PPV) for the diagnosis of HF. NT-proBNP (cut-off point 1000 pg/ml) has 92% sensitivity, 89% specificity, 86% NPV and 90% PPV. Boston modified criteria have 85% sensitivity, 86% specificity, 80% NPV and 90% PPV. Comparing the three methods, we found significant differences between ultrasound sign vs NT-proBNP (P<0.05) and Boston modified criteria (P<0.05). Combination of ultrasound sign and NT-proBNP has 100% sensitivity, 100% specificity, 100% NPV and 100% PPV. With ultrasound we can exclude HF in patients with pulmonary related dyspnea who have positive NT-proBNP (> 1000 pg/ml) and previous history of HF. Conclusions: Ultrasound comet tail sign alone or in combination with NT-proBNP has a high diagnostic accuracy in differentiating between acute HF and COPD/asthma causes of acute dyspnea in prehospital emergency setting.

CIRKADIJSKI DNEVNO-NOČNI BIOLOŠKI RITMI IN VARIABILNOST FREKVENCE SRCA

Opis: V diplomskem delu je opisano kaj je variabilnost frekvence srca (HRV). Opisali smo povezavo med HRV in različnimi boleznimi. Ugotovili smo kako se HRV spreminja v dnevno-nočnem času. Raziskava je vključevala 5 zdravih preiskovancev starih med 21 in 34 let od tega sta bili 2 osebi ženskega spola in 3 moškega. Prostovoljcem smo namestili aparat za 24 urno snemanje EKG-ja (Holter monitor) in posneli 120 ur podatkov oziroma 502.587 utripov srca. S pomočjo metod časovne in frekvenčne domene smo izračunali HRV. Rezultati so pokazali da preiskovanci, ki so imeli nemiren ali zamaknjen spanec so imeli tudi odstopanja v vrednosti cirkadijskega indeksa. Ugotovili smo tudi, da so bili vzroki za znižane vrednosti standardnega odklona vseh RR intervalov (SDNN) stresne situacije. Potrdili smo, da se ob fizični aktivnosti aktivira simpatično živčevje in zmanjša aktivnost parasimpatičnega živčevja in s tem se tudi poveča srčni utrip. Med počitkom po vadbi se srčni utrip vrne na izhodišče kar je posledica parasimpatične reaktivacije. Z neinvazivno metodo HRV smo prepoznali dejavnike tveganja za nastanek kariovaskularnih obolenj.
