UPORABA ENOTE HELIKOPTERSKE NUJNE MEDICINSKE POMOČI PRI OBRAVNAVI BOLNIKOV Z AKUTNIM KORONARNIM SINDROMOM

Abstract: V diplomskem delu smo predstavili enoto HNMP v Sloveniji, njeno delovanje in zgodovino. Obravnavali smo učinkovost njenega delovanja na vzorcu 252 bolnikov z akutnim koronarnim sindromom, ki jih je enota oskrbela po principih sodobnih smernic oskrbe takšnih bolnikov in prepeljala v prvih petih letih delovanja službe (16.7.2003 — 16.7.2008). Uporabili smo deskriptivno metodo s študijem literature za področje AKS. Uporabili smo retrospektivno metodo (študija izpolnjenih protokolov intervencij). Podatke smo analizirali (grafi). Prepeljanih je bilo 252 bolnikov z AKS, od tega 139 (55,15%) bolnikov z STEMI, 10 bolnikov (3.96%) z nestabilno angino pektoris, in 12 bolnikov (4,76l%) z NSTEMI. Za 91 bolnikov (36,11%) se zdravniki niso natančno opredelili o vodilni diagnozi. Ugotovili smo skupno število prepeljanih bolnikov in razloge za intervencijo, izračunali povprečne odzivne čase enote HNMP glede na prevoznika, skupne povprečne čase od klica do prihoda do bolnika, čas od klica do prihoda z bolnikom v bolnišnico ter čas od pristanka na pristajališču Roška do prihoda v bolnišnico z reševalnim vozilom. Na zemljevidu Slovenije smo grafično prikazali kraje posredovanja enote HNMP. Preučevali smo oskrbo bolnikov z akutnim koronarnim sindromom. Ugotovili smo, da nudi služba HNMP dobro oskrbo bolnikom, da so kadri enote na visokem nivoju, da je oprema, ki jo uporabljajo ustrezna in sodobna, in da služba HNMP skrbi za stalno dodatno izobraževanje svojega kadra. Ugotovili smo tudi pomanjkljivosti pri umeščenosti službe HNMP v Sloveniji, omejenost na dnevni čas delovanja, omejen način aktivacije, dolgi časi aktivacije in do prihoda bolnika v zdravstveno ustanovo, izguba časa zaradi neurejenega helioporta v Univerzitetnem kliničnem centru Ljubljana in neurejenega dispečerstva v zdravstvu. Ugotovili smo, da enota nima stalne posadke prevoznika, da se dnevno menja tip helikopterja s katerim posreduje, da so vsi helikopterji nenamenski in omejeno uporabni, in da ena enota v Sloveniji ni dovolj učinkovita za vse prebivalce Slovenije.

GOLJUFIJA NA ŠKODO EVROPSKIH SKUPNOSTI

Abstract: Kaznivo dejanje goljufije na škodo Evropskih skupnosti je Republika Slovenija v svoj pravni red uvedla z novim Kazenskim zakonikom, ki je v veljavi od 1. novembra 2008. S tem je izpolnila še zadnji del obveznosti, ki jih je sprejela s tem, ko je z zakonom ratificirala Konvencijo o zaščiti finančnih interesov Evropskih skupnosti, vključno z njenimi protokoli. Na podlagi navedene konvencije so bili v nedavni preteklosti sprejeti številni praktični ukrepi v boju zoper goljufije, ki imajo za posledico oškodovanje proračunskih sredstev Evropske unije. Ustanovljen je bil Evropski urad za boj proti goljufijam — OLAF. V vseh državah članicah podpisnicah konvencije delujejo operativne službe AFCOS, ki sodelujejo s tem uradom. Sprejetje in uveljavitev Lizbonske pogodbe, ki ustanavlja še tesnejšo zvezo med državami članicami, ponuja še dodatne ukrepe, ki se lahko sprejmejo na tem področju. Pravno urejanje na področju kazenskega prava bo mogoče tudi preko uredb in direktiv, kar bo olajšalo unifikacijo prava na tem področju. Prav tako je pomembna novost ustanovitev Evropskega javnega tožilstva, čigar naloga bo v prvi vrsti prav varovanje skupnih finančnih interesov. Prav tako se v prihodnosti pričakuje uveljavitev t.i. Corpus Iurisa kazenskopravnih določb, ki bo prav tako urejal predmetno tematiko. Z vsemi navedenimi ukrepi se nudijo realne možnosti za izgradnjo trdnega sistema javnih financ v okviru Evropske unije, to pa vodi k tesnejši nadnacionalni zvezi in k večjemu zaupanju evropskih državljanov v njene institucije.

Numerical analysis of the crack growth in a high loaded bolt connection

Abstract: The paper deals with the research on the crack growth in a bolt connection of a lug for crane counter weight bars. Counter weight bars are structural elements subjected to very heavy loads and therefore need special attention. The main purpose of this research is to determine the number of load cycles required for a crack to propagate from initial to criticakl crack length, whenthe final failure can be expected to occur. All required material parameters and experimental results were determined in our previous research. The influence of the initial crack siye upon the remaining life of the lug is reserched numericallu by means of FE analysis and analytically by use of the corrected analystical model.

QoS-oriented design of embedded systems with specification PEARL

Abstract: Only recently have methodical tools adequate to design real-time systems been formally introduced in design methodologies. Naturally, they were present fromthe beginning, but due to the large diversity of embedded systemsć areas of deployment, specially dedicated formalisms have been developed and used. High-level language programming and integration of modeling formalisms into design methods eased the development of more complex real-time applications. With the emerging object-oriented programming languages and design methods, their integration into larger information systems has become more transparent. It was the UML methodology, however, which eventually merged also the design methods and concepts of real-time systems into a consistent whole. It took a large consortium and a long process to persuade industry of the benefits the new integral methodology can offer. On the other hand, there are some trade-offs, and there are some features not completely covered, yet. Here, a different, more straightforward approach to program and design (embedded) real-time systems is presented. Since it emerged from the real-time community, it includes most features relevant there. Independent of the UML profile for schedulability, performance and time specification, a profile was devised for use in PEARL-oriented UML design. The strengths of the mentioned language and design methods for QoS-oriented design of (embedded) real-time systems are emphasised throughout this article.

The ecological motivation or tourists as a determinant of the tourists' loyalty

Abstract: Despite the significant impact of environment on the attractiveness of tourist destination, environmental stewardship is not always the top priority in the hotel industry as one of the most energy-intensive sectors of the tourism industry which contributes to the increasing quantity of air pollutants. Therefore, this study investigates the impact of ecological pull motives and service quality on the tourists' loyalty to eco-hotels. The tourists spent their holidays in Austrian hotels with ecolabel and in Slovenian hotels were targeted and a total of 611 tourists participated in the study. The study reveals that the perceived service quality and ecobehaviour of hotel staff are the main determinants of the tourists' loyalty. The comparison analysis between Austrian and Slovenian hotels indicates that the guests of Austrian hotels with ecolabel are more loyal and ecologically motivated than guests of Slovenian hotels without ecolabel.

Numerical calculation of bending fatigue life of thin-rim spur gears

Abstract: Mechanical elements subjected to cyclic loading have to be designed against fatigue. The main goal of the presented analysis was to determine the the bending fatigue life prediction of thin rim spur gears. The service life is divided into the initiation phase of the damage accumulation and the crack growth, resprcitively. The analysis has been performed usig FEM and BEM. The continuum mechanics approach is used for the prediction of the fatigue life initiation phase, where the basic fatigue material parameters are taken into account. The evaluation of the structure remaining life with initial crack the linear-elastic fracture mechanics LEFM principles are applied.

Review of mathematical and experimental models for determination of service life of gears

Abstract: To confirm mathematical models for the determination of service life of gears we developed different nonstandard test pieces and used a combination of mixed experimental methods on them. The aim of these combinations was to obtain as complete information as possible about the individual influences and to detrmine the interaction between different fracture mechanics magnitudes. In this way we confirmed the models as a whole and also determined some physical interpretations in models. With this we were able to ensure that the presented model is not purely a mathematical model.

Evaluation of the service life of gears in regrad to surface pitting

Abstract: A computational model for determining the service life of gear teeth flanks in regard to surface pitting is presented. The model considers the material fatigue process leading to pitting, i.e. the conditions required for the short fatigue crack propagation originating from the initial crack in a single material grain. In view of small crack lengths observed in surface pitting, the simulation takes into account the short crack growth theory. The stress field in the contact area and the required functional relationship between thestress intensity factor and the crack length are determined by the finite element method. An equivalent model of two contacting cylinders is used for numerical simulations of crack propagation in the contact area. On the basis of numerical results, and with consideration of some particular material parameters, the probable service life period of contacting surfaces is estimated for surface curvatures and loadings that are most commonly encountered in engineering practice.
