Working conditions and satisfaction with working conditions among Slovenian family medicine trainees : a cross-sectional study

Opis: Introduction: In order to achieve a high standard in training programmes for future family medicine specialists, it is essential to have good tutors with well-organised family medicine practices. Proper working conditions for young doctors are essential for their satisfaction and future professional development. The aim of our study was to check the current working conditions of family medicine trainees in the practical modular part of the training programme in Slovenia, and to determine their satisfaction with working conditions. Methods: A cross-sectional study was conducted. The data was collected through a questionnaire distributed to 105 family medicine trainees undergoing the practical modular part of their training programme. Results: The study showed that the following 7 out of 25 organisational and labour law factors are significantly associated with a trainee's general satisfaction with working conditions: the location where work with patients takes place, the privacy of the premises, the accessibility of the main tutor, a constant patient population, suitable places for rest, paid out-of-hours substitutions, and appropriate pay grade. Conclusion: The results we obtained can be used to address certain aspects of trainee's working conditions in Slovenia that need improvement. By determining which working conditions significantly affect a trainee's satisfaction, we have the opportunity to modify these conditions and thereby improve the training programme. This could result in a less stressful and more efficient residency programme.

Zdravljenje depresije z metodami stimulacije možganov

Opis: Uvod: Depresija predstavlja pomemben izziv za javno zdravje. Ta zmanjša kakovost življenja, povzroča invalidnost, sočasne bolezni in umrljivost. V nekaterih primerih ta kljub vsemu ostaja težko obvladljiva, zato so se razvile dodatne možnosti zdravljenja, kot so metode stimulacije možganov. Ti inovativni pristopi imajo sposobnost ciljnega vpliva na nevrotransmiterske poti in nevronsko aktivnost ter privedejo do izboljšanja njenih simptomov. Metode: V okviru zaključnega dela smo izvedli sistematičen pregled, analizo in sintezo literature s področja zdravljenja depresije z metodami stimulacije možganov. Uporabljena je bila opisna deskriptivna metoda znanstvene in strokovne literature. Literaturo smo iskali v podatkovnih bazah: PubMed, CINAHL Ultimate, Science Direct in Google Učenjak. Rezultati: Analizirali smo deset člankov, ki so ustrezali izbranim merilom, ki so se nanašali na raziskovalno vprašanje, tip raziskave in omejitve raziskave. Ugotovili smo, da so v vseh raziskavah potrdili pozitivne učinke različnih metod stimulacije možganov na izboljšanje simptomov depresije pri bolnikih. Na osnovi virov smo ugotovili, da se njena učinkovitost razlikuje glede na stopnjo razvitosti depresije in glede njene kombinacije zdravljenja. Razprava in zaključek: Analiza člankov je pokazala poudarek na duševnem zdravju v družbi, saj ta povzroča številne nevšečnosti. Ker je ta lahko težko obvladljiva, se razvijajo nove metode zdravljenja. Številne raziskave so pokazale pozitivne učinke metod stimulacije možganov.

The Careful Assessment tool for managing patients with medically unexplained symptoms - the experience of Slovenian family medicine trainees : a qualitative study

Opis: ntroduction Primary care physicians use various tools and methods to identify medically unexplained symptoms (MUS). The main purpose of our study is to determine the views of Slovenian family medicine trainees (FMT) about using the “Careful Assessment” tool for managing patients with MUS. Methods A qualitative study using open survey questions focused on the experience of family medicine trainees in managing patients with MUS. The sample consisted of surveys from 184 family medicine trainees. These trainees analysed a total of 702 patients with MUS. Manual coding was used for quantitative content analysis. Results In the coding process, 49 codes were developed that included broader research fields about using the “Careful Assessment” tool for managing patients with MUS. The codes were grouped into four theoretically grounded, logical categories in accordance with the elaborated theoretical concept: multi-purpose utility; improved patient management; in-depth knowledge and new skills; and patient response. Conclusion The study demonstrated that, in the view of Slovenian FMT, the “Careful Assessment” tool has multi-purpose utility. The study showed that FMT felt that this tool helps them in systematic patient management. Their opinion is that it helps them establish a trusting relationship with patients, which is a precondition for providing further treatment.

Uporaba in poznavanje kognitivno vedenjske terapije v psihiatrični zdravstveni negi

Opis: Kognitivno vedenjska terapija (KVT) je kolaborativni pristop, ki pomaga pacientom prepoznati disfunkcionalne misli in vedenjske vzorce ter jih zamenjati z bolj primernimi. Izkušnje iz otroštva izoblikujejo človekova prepričanja o samem sebi ter o svetu, ki ga obdaja. Takšna prepričanja so jedro njegove percepcije okolice, ki je lahko pozitivna ali negativna. KVT je pristop, ki se v tujini prakticira na vseh področjih zdravstvene nege in ki ga poleg psihoterapevtov in psihiatrov izvajajo tudi diplomirane medicinske sestre. Namen zaključnega dela je bil raziskati samooceno uporabe in poznavanja KVT pri zaposlenih na Oddelku za psihiatrijo.

The effect of an educational intervention in family phisicians on self-rated quality of life in patients with medically unexplained symptoms

Opis: Introduction: Medically unexplained symptoms (MUS) are very common in family medicine, despite being a poorly-defined clinical entity. This study aimed to evaluate the effect of an educational intervention (EI) on self-rated quality of life, treatment satisfaction, and the family physician-patient relationship in patients with MUS. Methods: In a multi-centre longitudinal intervention study, which was performed between 2012 and 2014, patients were asked to rate their quality of life, assess their depression, anxiety, stress and somatisation, complete the Hypochondriasis Index, the Medical Interview Satisfaction Scale and the Patient Enablement Instrument for assessing the physician-patient relationship, before and after the EI. Results: The mean values before and after the intervention showed that after the EI, patients with MUS gave a lower (total) mean rating of their health issues and a higher rating of their quality of life, and they also had a more positive opinion of their relationship with the physician (p<0.05). However, there were no differences in the (total) rating of treatment satisfaction before and after the EI (p=0.423). Significant differences in the symptoms in patients with MUS before and after the intervention were confirmed for stress, somatisation and hypochondriasis (p<0.05). Conclusions: It could be beneficial to equip family physicians with the knowledge, skills and tools to reduce hypochondriasis and somatisation in MUS patients, which would improve patients’ self-rated health status.

Uporaba komplementarnih in alternativnih metod zdravljenja med polnoletnimi prebivalci Slovenije

Opis: Purpose: The aim of our research was to determine how many people use complementary and alternative medicine (CAM) and how the use of CAM depends on the gender, age, education, and the living environment of each individual person. We wanted to determine the level of fondness for the providers of CAM and define the most popular alternative medicinal preparations. Methods: The cross-sectional study was based on an anonymous questionnaire sent to the houses of a randomly selected sample (N=1000) of adults from the Republic of Slovenia in April 2009. The sample was selected using the telephone book. The data was statistically analysed using the chi-square method and t-test, depending on the nature of the variables. Results: The survey was returned and answered correctly by 410 (41.0%) subjects. According to the results, complementary and alternative medicine was used by 9.0% of the subjects. The use of CAM was independent of age, gender, education, and the living environment. According to the point scale, homeopathy was trusted by 16.9% of people, followed by healers who were trusted by 11.0%. From a statistical point of view, homeopaths were much more trusted by people who had completed a secondary education. The most popular medicinal preparations were herbs and different types of tea (74.4%), followed by vitamins and minerals (69.0%). Homeopathic preparations were used by 4.6% of people. Conclusion: We confirmed a 9.0% presence of alternative medicine in Slovenia, not taking into account herbal medicines, treatments with herbal preparations or tea, and acupuncture. We also have established that 11.0% of the population trust healers and only 16.9% of the population trust homeopaths.
