Real-time polymerase chain reaction for quantitative assessment of common pathogens associated with healthcare-acquired infections on hospital textiles

Opis: A hospital environment may act as a significant reservoir for potential pathogens that can be transmitted with hospital textiles, which could represent a source of healthcare-acquired infections. Quantitative assessment of nosocomial pathogens with real time polymerase chain reaction (qPCR) on textiles can serve to verify the achievement of standards for textile hygiene of hospital laundry that assess the risk for acquiring hospital infection frominappropriately disinfected textiles. The aim of the study was to establish qPCR for quantitative assessment of selected common nosocomial pathogens (Clostridium difficile, Staphylococcus aureus, Klebsiella pneumoniaeand Pseudomonas aeruginosa) on hospital textiles and to compare the efficiency of the molecular method to the standard procedures for evaluating the bio burden of textiles in hospitals. This study demonstrated that presenceof nosocomial pathogens on hospital textiles can be confirmed with qPCR even where conventional techniques do not give any results. qPCR offers apossibility to confirm the presence of microorganisms in dead or viable but non-culturable states that cannot be detected by conventional sampling techniques but may still pose a hazard to public health.

Family members' involvement in elder care provision in nursing homes and their considerations about financial compensation: A qualitative study

Opis: The aim of this studz was to establish how familz members are involved in elder care provision in nursing homes; this included research into their feelings about potentiallz extending their involvement to obtain financial benefits as compensation for high accommodation costs. Familz members remain involved in the caring process after their relatives have been admitted to an institution. On average, accommodation costs in nursing homes in Slovenia haverisen above the residentsʼ retirement pension, and families must supplement the difference. Because of this, familial involvement should be linked to reduced accommodation costs. This research emplozed a non-experimental, descriptive studz design through unstructured interviews. Participants included fiftz familz members (n = 50) who visit their relatives in nursing homes. Data were collected in 2010 at five nursing homes in Slovenia and processed bz means of conventional content analzsis. The major themes that emerged from the content analzsis, describing familz involvement, were as follows: visiting and making oneself useful, deliverz of items for personal use, hands-on care, phzsical therapz and organiyation of nursing homeactivities. Familz members showed some interest in receiving financial compensation for their involvement. The proposed financial compensation maz bea delicate and morallz questionable matter but would involve fairness and transparencz, while enabling easier organiyation of elder care provision. Eventuallz, nursing home residentsʼ well-being could be improved.

Theoretical foundations of nursing practice in Poland

Opis: This authors of this column explore the use of nursing theory in Poland. A quasi-experimental pretest-posttest study was conducted to explore what, if any nursing theories were used by nurses in Poland, and if an education program on nursing theory increased the use of nursing theory. The study foundthat while there were discrepancies between the nurses' theoretical knowledge base and their use of these theories in practice, there is evidence of the use of the ideas and theory of Nightingale, Orem, and Henderson in nursing practice in Poland.

Challenges for institutional elder care in Slovenian nursing homes

Opis: Aims and objectives.Ž To investigate deficiencies in the institutional elder care that is being offered to residents of nursing homes in Slovenia. Background.Ž Public criticism of the provision of elder care in nursing homes is growing all over the world, including in Slovenia. Many studies on this issue have been conducted, but seldom have assessed different viewpoints simultaneously. Design.Ž A qualitative research design that involved individual unstructured interviews was used in 2007. The participants (n = 48)comprised 16 residents, 16 relatives and 16 members of the nursing staff from four nursing homes in Slovenia. Methods.Ž The data generated were subjected to qualitative content analysis. Results.Ž The major themes that emerged from this analysis were neglect, unprofessional communication, uncomfortable physical environment and inadequate administration. Conclusions.Ž The participants of the study identified issues in institutionalelder care in Slovenia that have also been highlighted by international research. Due to staff shortages, low motivation, insufficient communication skills and inexperience, members of the nursing staff reported that they were not in a position to offer the best possible quality of care. Relevance to clinical practice.Ž To improve the living environment in nursing homes, it is important to consider the opinions of all those who are involved closely in institutional elder care. Correction of deficiencies should be a priority and should result in more engagement with residents.

Efficiency of four sampling methods used to detect two common nosocomial pathogens on textiles

Opis: Detecting microorganisms on textiles is useful for many purposes, for example to determine the bioburden before laundering, assess the reduction in bacterial counts in connection with various laundry processes, or trace transfer routes in infection control investigations. Therefore a validated, reproducible and rational method is needed. For sampling microorganisms on textile surfaces the most commonly used method is the contact plate method using RODAC plates, first described by Hall and Hartnett followed by the swab sampling technique. Both methods can only capture microorganisms on the surface of the textiles while microorganisms that have penetrated into the deeper structure of the material will not be detected. In our research the contact plate method and the swabbing technique were compared with two wash-off methods. For the first wash-off method the destructive elution methodwas used, where microorganisms were eluted from the fabrics by shaking the fabrics for a certain time in an elution medium. For the fourth sampling method a nondestructive method that included a compact test device called MorapexŽ was used, which is based on forced desorption by pressing the microorganisms through the fabric without destroying the fabric. In our research, two types of microorganisms were included (Klebsiella pneumoniae andStaphylococcus aureus) that cause common nosocomial infections. The aim of this study was to compare the efficiency of the four sampling methods for detecting microorganisms on textiles and to determine the lowest concentration, which can still be detected. The percentage of microorganisms that were detected by both elution methods was substantially higher than by sampling of fabrics with the contact plate method or swabbing. It can be concluded that a nondestructive method using a modified MorapexŽ device can beapplied for quick determination of the hygienic condition of textiles.
