The role of visualization in estimating cardiovascular disease risk : scoping review

Opis: Background: Supporting and understanding the health of patients with chronic diseases and cardiovascular disease (CVD) risk is often a major challenge. Health data are often used in providing feedback to patients, and visualization plays an important role in facilitating the interpretation and understanding of data and, thus, influencing patients’ behavior. Visual analytics enable efficient analysis and understanding of large datasets in real time. Digital health technologies can promote healthy lifestyle choices and assist in estimating CVD risk. Objective: This review aims to present the most-used visualization techniques to estimate CVD risk. Methods: In this scoping review, we followed the Joanna Briggs Institute PRISMA-ScR (Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews) guidelines. The search strategy involved searching databases, including PubMed, CINAHL Ultimate, MEDLINE, and Web of Science, and gray literature from Google Scholar. This review included English-language articles on digital health, mobile health, mobile apps, images, charts, and decision support systems for estimating CVD risk, as well as empirical studies, excluding irrelevant studies and commentaries, editorials, and systematic reviews. Results: We found 774 articles and screened them against the inclusion and exclusion criteria. The final scoping review included 17 studies that used different methodologies, including descriptive, quantitative, and population-based studies. Some prognostic models, such as the Framingham Risk Profile, World Health Organization and International Society of Hypertension risk prediction charts, Cardiovascular Risk Score, and a simplified Persian atherosclerotic CVD risk stratification, were simpler and did not require laboratory tests, whereas others, including the Joint British Societies recommendations on the prevention of CVD, Systematic Coronary Risk Evaluation, and Framingham-Registre Gironí del COR, were more complex and required laboratory testing–related results. The most frequently used prognostic risk factors were age, sex, and blood pressure (16/17, 94% of the studies); smoking status (14/17, 82%); diabetes status (11/17, 65%); family history (10/17, 59%); high-density lipoprotein and total cholesterol (9/17, 53%); and triglycerides and low-density lipoprotein cholesterol (6/17, 35%). The most frequently used visualization techniques in the studies were visual cues (10/17, 59%), followed by bar charts (5/17, 29%) and graphs (4/17, 24%). Conclusions: On the basis of the scoping review, we found that visualization is very rarely included in the prognostic models themselves even though technology-based interventions improve health care worker performance, knowledge, motivation, and compliance by integrating machine learning and visual analytics into applications to identify and respond to estimation of CVD risk. Visualization aids in understanding risk factors and disease outcomes, improving bioinformatics and biomedicine. However, evidence on mobile health’s effectiveness in improving CVD outcomes is limited.

Climate change and agriculture management : Western Balkan region analysis

Opis: Background: This paper aims to analyze the possibilities of the agricultural sector of the Western Balkan to assess compliance with the European Green Deal, which provides for the implementation of activities, which should enable the transition to sustainable agriculture and climate change mitigation. This paper is among the first to present the causality of agriculture and climate change (status, mitigation, and perspectives) in general and in light of the European Green Deal for the Western Balkan territory. Main text: Agricultural production is a leading industry in the Western Balkan. Climate change and predictions that temperatures will increase by 4 °C in the coming decades pose a risk not only to agricultural production but also to the safety of the population, because agriculture is the main source of income for a significant part of it. Uncontrolled floods and droughts caused by climate change are a particular danger for agriculture and human existence. This paper demonstrates that agriculture in the WB can be considered critically affected by climate change. Conclusions:Unless appropriate measures are taken and risk management for water resources and agriculture is improved, there will be a further decrease in precipitation and an increase in dry days by 20%. Such a scenario endangers not only the already vulnerable climate sustainability and biodiversity of the region but also the existence of a population employed in agriculture and the contribution of the agricultural sector to the gross domestic product. However, future planning based on the Common Agriculture Policy (CAP) and European Green Deal, the adoption of a related regulatory framework, the establishment and regular monitoring of supporting financing mechanisms, regional cooperation, and improving risk management (with emphasis on the local level) can mitigate the present impact and decrease the expected negative impact of climate change on agriculture and biodiversity in the WB region.

Targeting cardiovascular risk factors through dietary adaptations and caloric restriction mimetics

Opis: The average human life expectancy continues to rise globally and so does the prevalence and absolute burden of cardiovascular disease. Dietary restriction promotes longevity and improves various cardiovascular risk factors, including hypertension, obesity, diabetes mellitus, and metabolic syndrome. However, low adherence to caloric restriction renders this stringent dietary intervention challenging to adopt as a standard practice for cardiovascular disease prevention. Hence, alternative eating patterns and strategies that recapitulate the salutary benefits of caloric restriction are under intense investigation. Here, we first provide an overview of alternative interventions, including intermittent fasting, alternate-day fasting and the Mediterranean diet, along with their cardiometabolic effects in animal models and humans. We then present emerging pharmacological alternatives, including spermidine, NAD+ precursors, resveratrol, and metformin, as promising caloric restriction mimetics, and briefly touch on the mechanisms underpinning their cardiometabolic and health-promoting effects. We conclude that implementation of feasible dietary approaches holds the promise to attenuate the burden of cardiovascular disease and facilitate healthy aging in humans.

The impact of seven macroprudential policy instruments on financial stability in six euro area economies

Opis: The aim of this paper is to investigate whether macroprudential policy instruments can influence the credit growth rate and hence financial stability. We use a fixed effects panel regression model to test the following hypothesis for six euro area economies (Austria, Finland, Germany, Italy, Netherlands and Spain) during time span 2010 Q3 to 2018 Q4: “Macroprudential policy instruments (degree of maturity mismatch; interbank loans as a percentage of total loans; leverage ratio; non-deposit funding as a percentage of total funding; loan-to-value ratio; loan-to-deposit ratio; solvency ratio) enhance financial stability, as measured by credit growth”. Our empirical results suggest that the degree of maturity mismatch, non-deposit funding as a percentage of total funding, loan-to-value ratio and loan-to-deposit ratio exhibit the predicted impact on the credit growth rate and therefore on financial stability. On the other hand, interbank loans as a percentage of total loans, leverage ratio, and solvency ratio do not exhibit the expected impact on the response variable. Since only four regressors (out of seven) have the signs predicted by our hypothesis, we can only partly confirm it.

Exploring an infrastructure investment methodology to risk mitigation from rail hazardous materials shipments

Opis: Railroad is one of the primary modes to transport hazardous materials (hazmat) in North America. For instance, Canadian railroads carried around 50 million tons of hazmat in 2018. Given the inherent danger of trains carrying hazmat, this study aimed at exploring a novel way towards mitigation of the associated risk. This study sought to investigate whether proper rail track infrastructure investment can mitigate the risk from hazmat shipments. To this end, a methodology was developed and then applied to the Canadian railroad network. The proposed three-step methodology captured the differing perspectives of rail carriers and regulatory agencies, and entailed (1) ascertaining the risk-level of various yards and links in the given railroad network, (2) specifying potential candidates for infrastructure investment, and (3) finding the optimum set of investment decisions. The proposed methodology was then applied to the Canadian railroad network to demonstrate that significant risk-reduction can beachieved by adding alternative rail-links around the riskiest locations (i.e. the network hot-spots), and also to show that risk-reduction function is non-linear with non-monotonous behavior. The study showed the possibility of significant hazmat risk reduction through alternative rail-links that could take traffic away from the network hot-spots. The methodology and the resultsfrom the Canadian case can be used by railroad companies and policy makers to estimate the value of potentially risk-reducinginfrastructure investments.

Towards productive and ergonomic order picking : multi-objective modeling approach

Opis: The logistics sector should strive for sustainability alongside productivity by protecting its order pickers' health and welfare. Existing storage assignment models are mainly based on the criterion of order picking time and, to a lesser extent, the human factor. In the paper, a solution to a storage assignment problem using a multi-objective model based on binary integer linear programing is presented by developing a solution that considers order picking time, energy expenditure and health risk. The Ovako Working Posture Assessment System (OWAS) method was used for health risk assessment. The downside of solely health risk-optimization is that the average order picking time increases by approximately 33 % compared to solely time-optimization. Contrary to this, the developed multi-objective function emphasizing time has proven to be promising in finding a compromise between the optimal order picking time and eliminating work situations with a very-high risk for injuries. Its use increases the time by only 3.8 % compared to solely time-optimization while significantly reducing health risk.

Multilingual framework for risk assessment and symptom tracking (MRAST)

Opis: The importance and value of real-world data in healthcare cannot be overstated because it offers a valuable source of insights into patient experiences. Traditional patient-reported experience and outcomes measures (PREMs/PROMs) often fall short in addressing the complexities of these experiences due to subjectivity and their inability to precisely target the questions asked. In contrast, diary recordings offer a promising solution. They can provide a comprehensive picture of psychological well-being, encompassing both psychological and physiological symptoms. This study explores how using advanced digital technologies, i.e., automatic speech recognition and natural language processing, can efficiently capture patient insights in oncology settings. We introduce the MRAST framework, a simplified way to collect, structure, and understand patient data using questionnaires and diary recordings. The framework was validated in a prospective study with 81 colorectal and 85 breast cancer survivors, of whom 37 were male and 129 were female. Overall, the patients evaluated the solution as well made; they found it easy to use and integrate into their daily routine. The majority (75.3%) of the cancer survivors participating in the study were willing to engage in health monitoring activities using digital wearable devices daily for an extended period. Throughout the study, there was a noticeable increase in the number of participants who perceived the system as having excellent usability. Despite some negative feedback, 44.44% of patients still rated the app’s usability as above satisfactory (i.e., 7.9 on 1–10 scale) and the experience with diary recording as above satisfactory (i.e., 7.0 on 1–10 scale). Overall, these findings also underscore the significance of user testing and continuous improvement in enhancing the usability and user acceptance of solutions like the MRAST framework. Overall, the automated extraction of information from diaries represents a pivotal step toward a more patient-centered approach, where healthcare decisions are based on real-world experiences and tailored to individual needs. The potential usefulness of such data is enormous, as it enables better measurement of everyday experiences and opens new avenues for patient-centered care.

The Lyme borreliosis spatial footprint in the 21st century : a key study of Slovenia

Opis: After mosquitoes, ticks are the most important vectors of infectious diseases. They play an important role in public health. In recent decades, we discovered new tick-borne diseases; additionally, those that are already known are spreading to new areas because of climate change. Slovenia is an endemic region for Lyme borreliosis and one of the countries with the highest incidence of this disease on a global scale. Thus, the spatial pattern of Slovenian Lyme borreliosis prevalence was modelled with 246 indicators and transformed into 24 uncorrelated predictor variables that were applied in geographically weighted regression and regression tree algorithms. The projected potential shifts in Lyme borreliosis foci by 2050 and 2070 were calculated according to the RCP8.5 climate scenario. These results were further applied to developing a Slovenian Lyme borreliosis infection risk map, which could be used as a preventive decision support system.

Risk factors for systemic reactions in typical cold urticaria : results from the COLD-CE study

Opis: Background: Cold urticaria (ColdU), i.e. the occurrence of wheals or angioedema in response to cold exposure, is classified into typical and atypical forms. The diagnosis of typical ColdU relies on whealing in response to local cold stimulation testing (CST). It can also manifest with cold-induced anaphylaxis (ColdA). We aimed to determine risk factors for ColdA in typical ColdU. Methods: An international, cross-sectional study COLD-CE was carried out at 32 urticaria centers of reference and excellence (UCAREs). Detailed history was taken and CST with an ice cube and/or TempTest® performed. ColdA was defined as an acute cold-induced involvement of the skin and/or visible mucosal tissue and at least one of: cardiovascular manifestations, difficulty breathing, or gastrointestinal symptoms. Results: Of 551 ColdU patients, 75% (n=412) had a positive CST and ColdA occurred in 37% (n=151) of the latter. Cold-induced generalized wheals, angioedema, acral swelling, oropharyngeal/laryngeal symptoms, and itch of earlobes were identified as signs/symptoms of severe disease. ColdA was most commonly provoked by complete cold water immersion and ColdA caused by cold air was more common in countries with a warmer climate. Ten percent (n=40) of typical ColdU patients had a concomitant chronic spontaneous urticaria (CSU). They had a lower frequency of ColdA than those without CSU (4% vs 39%, p=0.003). We identified the following risk factors for cardiovascular manifestations: previous systemic reaction to a Hymenoptera sting, angioedema, oropharyngeal/laryngeal symptoms, and itchy earlobes. Conclusion: ColdA is common in typical ColdU. High-risk patients require education about their condition and how to use an adrenaline autoinjector.

Fall risk in adult family practice non-attenders : a cross-sectional study from Slovenia

Opis: Introduction: Not much is known about the fall risk among the adult population of those who rarely visit doctors. We wanted to determine the prevalence of increased fall risk in a population of family practice non-attenders and the factors associated with it. Methods: We included participants from family medicine practices in this cross-sectional study. To be included in the study, the participants had to be adults living in the community (home-dwelling people) who had not visited their chosen family physician in the last five years (non-attenders). The identification of the eligible persons was done through a search of electronic medical records, which yield 2,025 non-attenders. Community nurses collected data in the participants’ homes. The outcome measure was increased fall risk as assessed by the Morse fall scale: increased risk (≥25) vs. no risk. Results: The sample consisted of 1,945 patients (96.0% response rate) with a mean age of 60.4 years (range 20.5 to 99.7 years). An increased fall risk was determined in 482 or 24.8% (95% CI: [22.9, 26.8]) of the patients. The multivariate model showed a significant association of increased fall risk with higher age (p<0.001), lower systolic blood pressure (p=0.047), poor family function (p=0.016), increased risk of malnutrition (p=0.013), higher number of chronic diseases (p=0.027), higher pain intensity (p<0.001), lower self-assessment of current health (p=0.002), and higher dependence in daily activities (p<0.001). Conclusion: Non-attenders may have an increased risk of falling which depends on their health status and age. The inclusion of community nurses in primary healthcare teams could be of use not only to identify the non-attenders’ health needs, but also to better manage their health, especially the factors that were identified to be associated with greater fall risk.
