Management of children and adolescents with coeliac disease in selected Central European countries

Opis: Coeliac disease (CD) is a lifelong systemic autoimmune disorder elicited by gluten and related prolamines in genetically susceptible individuals and is one of the most common chronic diseases, affecting about 1% of the population. It has a very diverse clinical presentation, involving intestinal, extraintestinal and even asymptomatic presentations. Due to its genetic background, CD is more common among family members of affected individuals and is associated with a number of other conditions. The diagnosis of CD is traditionally based on the presence of a specific immune response and characteristic histological changes in the small intestinal mucosa. In the recently published European Society for Pediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN) guidelines, the initial diagnostic step is determination of CD-specific IgA autoantibodies against type-2 (tissue) transglutaminase. Positive autoantibodies imply a high probability of mucosal atrophy. To confirm the diagnosis, an upper endoscopy with multiple duodenal biopsies should be performed. However, ESPGHAN guidelines for the past eight years allow paediatric gastroenterologists to diagnose the disease without intestinal biopsy when certain criteria are met. The only available treatment for CD is a lifelong strict gluten-free diet, which should only be initiated after firm confirmation of the diagnosis. Patients have to avoid all gluten-containing products, including products from wheat and wheat cultivars, rye, barley, Khorasan wheat, triticale and bulgur. Owing to the high likelihood of cross-contamination, avoidance of oats is also recommended in many regions. Despite being one of the most common lifelong disorders, CD still remains undiagnosed for a long time in many adult and paediatric patients. Diagnostic delays in some regions exceed 10 years, which can have important adverse effects on patients’ health and quality of life. The aims of our study were to analyse the clinical presentation of CD in children and adolescents in selected Central European (CE) countries, to evaluate the compliance with current ESPGHAN guidelines for diagnosing CD in children and adolescents, to assess the diagnostic delays in children and adolescents with CD in selected CE countries, to identify possible regional differences, and to determine whether the clinical presentation or the diagnostic approach influence diagnostic delays. The study was carried out as a part of the Focus IN CD project with twelve project partners from five CE countries (Croatia, Germany, Hungary, Italy, and Slovenia). Paediatric gastroenterologists from the included regions were encouraged by the regional project partners to complete a web-based questionnaire, providing anonymised medical records of children and adolescents below 19 years of age who were diagnosed with CD in 2016. The questionnaire was translated into the languages of all project partners and focused on the onset of the CD-related symptoms, clinical presentation, diagnostic approach, and the results of all performed tests. The medical records of children and adolescents were then analysed, and the results compared regionally. We found that a non-classical clinical presentation and asymptomatic disease are becoming more common, and the classical clinical presentation with malabsorption syndrome less frequent in children and adolescents with CD. Opis: Novel coronavirus disease (COVID-19), named a pandemic by the WHO, is the current global health crisis. National and international collaboration are indispensable for combating COVID-19 and other similar potential outbreaks. International efforts to tackle this complex problem have led to remarkable scientific advances. Yet, as a global society, we can and must take additional measures to fight this pandemic. Undoubtedly, our approach toward COVID-19 was not perfect, and testing has not been deployed fast enough to arrest the epidemic early on. It is critical that we revise our approaches to be more prepared for pandemics as a united body by promoting global cooperation and commitment.

Research data and software for paper Forecasting COVID-19

Opis: The World Health Organization declared the coronavirus disease 2019 a pandemic on March 11th, pointing to the over 118,000 cases in over 110 countries and territories around the world at that time. At the time of writing this manuscript, the number of confirmed cases has been surging rapidly past the half-million mark, emphasizing the sustained risk of further global spread. Governments around the world are imposing various containment measures while the healthcare system is bracing itself for tsunamis of infected individuals that will seek treatment. It is therefore important to know what to expect in terms of the growth of the number of cases, and to understand what is needed to arrest the very worrying trends. To that effect, we here show forecasts obtained with a simple iteration method that needs only the daily values of confirmed cases as input. The method takes into account expected recoveries and deaths, and it determines maximally allowed daily growth rates that lead away from exponential increase toward stable and declining numbers. Forecasts show that daily growth rates should be kept at least below 5% if we wish to see plateaus any time soon - unfortunately far from reality in most countries to date. We provide an executable as well as the source code for a straightforward application of the method on data from other countries.

Developing police search strategies for elderly missing persons in Croatia

Opis: Purpose: This paper examines the distribution of elderly missing persons in the city of Zagreb per variables (age, sex of the missing person, social and marital status, employment, place and duration of disappearance, method of finding the missing person, its outcome, reasons for disappearance, etc.) needed for the design of the plan of the search for the missing person. Design/Methods/Approach: The paper uses data collected through a questionnaire with 417 variables and processed with certain basic statistical methods. The collected data are examined on a sample of 170 elderly missing persons in the city of Zagreb, and through characteristics of methods, measures and actions, together with the methods of search for elderly persons in cities, with special reference to Zagreb. Findings: The statistically established behavioural modes of certain groups of elderly people are analysed: persons with Alzheimer’s disease and persons with suicidal risk. The stated findings largely assist in planning and conducting the search for those persons, especially for missing person profiling and his/her possible movement since disappearance. The success of the search for the missing person presumes knowledge about the stated specifics by all searchers. It especially applies to the police in the formal, due to its sole duty and competence for the search for missing persons, as well as in the real sense where they conduct search measures and actions together with other participants adapted to real situations of disappearance of elderly persons. Research Limitations / Implications: The research has been limited by data available from the police bulletin of daily events, but this limitation was partially removed by a direct insight into the police files containing the case history. During the research, the legal and ethical regulations regarding personal data protection were strictly followed. Originality/Value: There is a number of researches on police search for the missing persons (UK, USA, Australia etc.), but few of them deal with elderly persons. This paper introduces a new offensive approach to the search for the missing person on a strategic level (missing person profiling with regard to specific variables, possible movement and routes, “wanderings”). New police procedures are suggested on a tactical level (e.g., stopping a person who disappeared in a certain area), but they include traditional police methods (car & regular patrols, police officers on bicycles, police search dogs, collecting information, conducting interviews, terrain search etc.). Introduction of the principle of partnership search for the missing person. Research results should be useful to police practice and their education.

Classification of perimetric data for supporting glaucoma diagnosis

Opis: The aim of the study: Glaucoma is a chronic, progressive and asymptomatic retinal disease which results in an irreversible visual field loss. The main objective of this Master’s thesis work was to study the applicability of classification techniques for supporting glaucoma diagnosis. Research Methodology: In this study perimetric data was obtained by SPARK strategy implemented in Oculus perimeters and provided by medical experts from the Hospital Universitario de Canarias (HUC). This data was used for constructing the feature vectors for the classification problem. Feature vectors of 66 values and feature vectors of 6 values were tested in the experiments. The proposed classification study attempted to: a) demonstrate that the studied classifiers were able to distinguish between “healthy” and “glaucomatous” eyes using only perimetric data, and b) analyse which feature vector design was the most suitable to accomplish this task. Results: The classification results showed that classifiers performed better on 6 than on 66 perimetry values, which demonstrated the suitability of the 6 points selected by the SPARK strategy and supported its use in medical field. Conclusion: In this study two remarkable findings for pattern recognition in perimetric data were obtained. Firstly, that reducing the dataset improved the efficiency of the studied classifier, and secondly, that simple pattern recognition models types were more efficient than complex ones.

Growth performance, productivity and diseases susceptibility of barley varieties in Slovenia within the Cobra project's site comparison

Opis: Different plant genotypes react differently in different climates. A field experiment was carried out to estimate the growth performance, productivity and diseases susceptibility of spring barley varieties in the Slovenian climate. We received some varieties, mainly of Nordic origin, from the Technical University of Denmark, a COBRA project partner, which were previously tested in estimated future climate in RERAF phytotron. Varieties of the highest grain yield (3,993 kg ha$^{-1}$ in ‘Evergreen’ to 5,146 kg ha$^{-1}$ in ‘Sebastian’) were rather shorter (58.7 cm to 67.1 cm) and mostly had the highest specific grain weight (54.3 to 58.6 kg 100 L$^{-1}$) and 1000-kernel weight (30.2 to 37.1 g). They developed 1,561 to 2,532 tillers m$^{-2}$ and 515 to 840 ears m$^{-2}$ and reached a heading stage between 13th and 25th of May. The tested varieties seem rather insusceptible to most common diseases, but susceptible to cereal leaf beetle attacks.

New perspectives for computer-aided discrimination of Parkinson's disease and essential tremor

Opis: Pathological tremor is a common but highly complex movement disorder, affecting ~5% of population older than 65 years. Different methodologies have been proposed for its quantification. Nevertheless, the discrimination between Parkinson's disease tremor and essential tremor remains a daunting clinical challenge, greatly impacting patient treatment and basic research. Here, we propose and compare several movement-based and electromyography-based tremor quantification metrics. For the latter, we identified individual motor unit discharge patterns from high-density surface electromyograms and characterized the neural drive to a single muscle and how it relates to other affected muscles in 27 Parkinson's disease and 27 essential tremor patients. We also computed several metrics from the literature. The most discriminative metrics were the symmetry of the neural drive to muscles, motor unit synchronization, and the mean log power of the tremor harmonics in movement recordings. Noteworthily, the first two most discriminative metrics were proposed in this study. We then used decision tree modelling to find the most discriminative combinations of individual metrics, which increased the accuracy of tremor type discrimination to 94%. In summary, the proposed neural drive-based metrics were the most accurate at discriminating and characterizing the two most common pathological tremor types.
