2. Expressing added value in food supply chainsNejc Zidar, Jernej Turk, Jernej Prišenk, 2023, original scientific article Abstract: Value-added food supply chains are chains that maintain positive social, environmental, and community values which are integrated into the production process from the primary producer to the final consumer, ensuring economic, social, and environmental sustainability, thus forming a sustainable agricultural and food production system. The evaluation of the value added in each chain is based on indicators, which are a tool to measure changes in the actual situation or to show performance trends in achieving the progress set by the objectives. The presented indicators are based on publicly available statistics and foreign literature that allow measuring value added in specific livestock chains at aggregate level (beef production, pork production and dairy production) and are based on publicly available statistics in the Republic of Slovenia. The need to produce indicators of value added in livestock food supply chains arises from the large differences in structural changes in the sector itself and in the development and economic performance of the various livestock industries. 5. Ranking as a procedure for selecting a replacement variable in the score predicting the survival of patients treated with curative intent for colorectal liver metastasesIrena Plahuta, Matej Mencinger, Iztok Peruš, Tomislav Magdalenić, Špela Turk, Aleks Brumec, Stojan Potrč, Arpad Ivanecz, 2023, original scientific article Abstract: Background and Objectives: The issue of a missing variable precludes the external validation of many prognostic models. For example, the Liverpool score predicts the survival of patients undergoing surgical therapy for colorectal liver metastases, but it includes the neutrophil–lymphocyte ratio, which cannot be measured retrospectively.
Materials and Methods: We aimed to find the most appropriate replacement for the neutrophil–lymphocyte ratio. Survival analysis was performed on data representing 632 liver resections for colorectal liver metastases from 2000 to 2020. Variables associated with the Liverpool score, C-reactive protein, albumins, and fibrinogen were ranked. The rankings were performed in four ways: The first two were based on the Kaplan-Meier method (log-rank statistics and the definite integral
between two survival curves). The next method of ranking was based on univariate and multivariate Cox regression analyses.
Results: The ranks were as follows: the radicality of liver resection (rank 1), lymph node infiltration of primary colorectal cancer (rank 2), elevated C-reactive protein (rank 3), the American Society of Anesthesiologists Classification grade (rank 4), the right-sidedness of primary colorectal cancer (rank 5), the multiplicity of colorectal liver metastases (rank 6), the size of colorectal liver metastases (rank 7), albumins (rank 8), and fibrinogen (rank 9).
Conclusions: The ranking methodologies resulted in almost the same ranking order of the variables. Elevated C-reactive protein was ranked highly and can be considered a relevant replacement for the neutrophil–lymphocyte ratio in the Liverpool score. These methods are suitable for ranking variables in similar models for medical research. 6. The learning curve of laparoscopic liver resection utilising a difficulty scoreArpad Ivanecz, Irena Plahuta, Matej Mencinger, Iztok Peruš, Tomislav Magdalenić, Špela Turk, Stojan Potrč, 2022, original scientific article Abstract: Background: This study aimed to quantitatively evaluate the learning curve of laparoscopic liver resection (LLR) of a single surgeon.
Patients and methods: A retrospective review of a prospectively maintained database of liver resections was conducted. 171 patients undergoing pure LLRs between April 2008 and April 2021 were analysed. The Halls difficulty score (HDS) for theoretical predictions of intraoperative complications (IOC) during LLR was applied. IOC was defined as blood loss over 775 mL, unintentional damage to the surrounding structures, and conversion to an open approach. Theoretical association between HDS and the predicted probability of IOC was utilised to objectify the shape of the learning curve.
Results: The obtained learning curve has resulted from thirteen years of surgical effort of a single surgeon. It consists of an absolute and a relative part in the mathematical description of the additive function described by the logarithmic function (absolute complexity) and fifth-degree regression curve (relative complexity). The obtained learning curve determines the functional dependency of the learning outcome versus time and indicates several local extreme values (peaks and valleys) in the learning process until proficiency is achieved.
Conclusions: This learning curve indicates an ongoing learning process for LLR. The proposed mathematical model can be applied for any surgical procedure with an existing difficulty score and a known theoretically predicted association between the difficulty score and given outcome (for example, IOC). 7. Sweet, fat and salty: snacks in vending machines in health and social care institutions in SloveniaUrška Rozman, Igor Pravst, Urška Pivk Kupirovič, Urška Blaznik, Primož Kocbek, Sonja Šostar-Turk, 2020, original scientific article Abstract: Vending machines in health and social care facilities are often the only possible choice for a quick snack for workers and visitors, in many cases providing unhealthy dietary choices. Our study aimed to analyse the variety and nutritional quality of foods available in vending machines placed in social and health care institution in Slovenia. The available snacks were quantitatively assessed, using traffic light profiling. The model used for nutrient profiling was that of the Food Standards Australia New Zealand (FSANZ). Vending machines in 188 institutions were surveyed, resulting in 5625 food-items consisting of 267 unique product labels. Sweet products dominate in vending machines offers (about 70%), while nuts and seeds (8.4%), yoghurts (2.1%), fruits (1.4%) and milk (0.3%) are present in a very small proportion or are not available at all. According to FSANZ, 88.5% of all displayed food items in vending machines can be considered as lower nutritional quality or less healthy products. The authors’ future activities will be focused on ensuring wider availability of healthy dietary choices and on including official guidelines in tender conditions for vending machines in health and social care institutions in Slovenia. 8. Kultura varnosti pri delu v operacijskih dvoranahTeodor Pevec, 2025, doctoral dissertation Abstract: Izhodišča in namen raziskave: Bolnikova varnost je postala glavna skrb izvajalcev in kreatorjev zdravstvenega sistema, saj se v Evropi varnostni zapleti zgodijo pri 8 % do 12 % vseh sprejetih bolnikov v bolnišnice.
Metode: Razvili smo slovenski vprašalnik odnosa do varnosti v operacijskih dvoranah in na
Rezultati: Slovenska verzija vprašalnika odnosa do varnosti v operacijski dvorani vsebuje 28 vprašanj in izkazuje primerne kriterije veljavnosti in zanesljivosti.
10. Effect of Lactobacillus spp. on adhesion, invasion, and translocation of Campylobacter jejuni in chicken and pig small-intestinal epithelial cell linesMaja Šikić Pogačar, Tomaž Langerholc, Dušanka Mičetić-Turk, Sonja Smole Možina, Anja Klančnik, 2020, original scientific article Abstract: Background : Campylobacter spp. are a major cause of bacterial food-borne diarrhoeal disease. This mainly arises through contamination of meat products during processing. For infection, Campylobacter spp. must adhere to epithelial cells of the mucus layer, survive conditions of the gastrointestinal tract, and colonise the intestine of the host. Addition of probiotic bacteria might promote competitive a dhesion to epithelial cells, consequently reducing Campylobacter jejuni colonisation. Effect of Lactobacillus spp. (PCS20, PCS22, PCS25, LGG, PCK9) on C. jejuni adhesion, invasion and translocation in pig (PSI cl.1) and chicken (B1OXI) small-intestine cell lines, as well as pig enterocytes (CLAB) was investigated.
Results : Overall, in competitive adhesion assays with PSI cl.1 and CLAB cell monolayers, the addition of Lactobacillus spp. reduced C. jejuni adherence to the cell surface, and negatively affected the C. jejuni invasion. Interestingly, Lactobacillus spp. significantly impaired C. jejuni adhesion in three-dimensional functional PSI cl.1 and B1OXI cell models. Also, C. jejuni did not translocate across PSI cl.1 and B1OXI cell monolayers when co-incubated with probiotics. Among selected probiotics, Lactobacillus rhamnosus LGG was the strain that reduced adhesion efficacy of C. jejuni most significantly under co-culture conditions.
Conclusion : The addition of Lactobacillus spp. to feed additives in livestock nutrition might be an effective novel strategy that targets Campylobacter adhesion to epithelial cells, and thus prevents colonisation, reduces the transmission, and finally lowers the incidence of human campylobacteriosis.
